EXHIBIT A SCOPE OF WORK 10. The term of this Agreement shall be until all Community Development Block Grant (CDBG) funds authorized by this Agreement are expended or this Agreement is terminated. 11. The CDBG funds approved under this Agreement shall only be used for the acquisition of the properties located at 22198 & 22200 Center Street, Castro Valley, California to be used to create 27 new construction units of affordable and supportive housing to serve the long- term housing, health and social service needs of 27 homeless and disabled families and individuals from Mid- and Southern Alameda County, including the City of San Leandro. 12. The Recipient shall follow all provisions and keep all records set forth in Exhibit D and Exhibit E of this Agreement. 13. The Recipient shall include the required provisions and language in all construction contracts for Work on the "Property ", signed during the term of this Agreement. 14. The Recipient shall request any changes to the list of Work or budget in writing. The City Manager may approve requests at his discretion. 15. Reporting Requirements: Subgrantee shall submit semi - annual activity reports by the last day of the month following the end of each semi - annual period. Such reports shall be in the format provided by the City and shall include for each activity: • total number of assisted households; • description of services provided; • total units of service completed; and • client data for each head of household: ethnicity, gender, income level, age, and disability. 16. Program Goals: Allied Housing, Inc. will coordinate placement and service efforts with referring agencies located in San Leandro, and other agencies, to ensure that placement and service is provided to eligible San Leandro residents.
