EXHIBIT A <br /> SCOPE OF WORK (REVISED) <br /> Term of Contract: July 1, 2010 — June 30, 2011 <br /> Agency: Girls Incorporated of Alameda County <br /> Address: 13666 East 14th Street <br /> San Leandro, CA 94706 <br /> Contact Person: Rebecca Cannon, Chief Mental Health Officer <br /> Phone: 510- 357 -5515 <br /> Project Name: Pathways Counseling Center's Family Strengthening Program <br /> PROJECT GOALS AND OBJECTIVES <br /> The Pathways Counseling Centers Family Strengthening Program will provide individual and family therapy services and Family <br /> Communication Workshops to enable individuals and families to address psychological and communication problems within their <br /> families to increase the likelihood that these problems do not escalate into parental abuse or adolescent acting -out behavior. The <br /> program will also provide family crisis intervention services, in which children and families can engage in order to develop <br /> communication skills and psychological capacity to overcome the current "hot° crisis of their families and reunite as a family. The <br /> program will provide respite care intervention for pre - delinquent and delinquent youth. Services will be delivered in Spanish and <br /> English. <br /> MAJOR ACTIVITIES <br /> Pathways services are: Individual and family therapy services as well as consultation services with other professionals involved in the <br /> cases (for example, teachers, school administrators, pediatricians, psychiatrists, etc.) <br /> Mental Health Clinicians will conduct clinical assessments (measured on a 10 -point scale conducted at: intake, subsequent 90 -day <br /> intervals, and termination). Participating family members will complete communication skills assessments as well as survey <br /> evaluations regarding quality of treatment services. <br /> Many additional services are delivered for San Leandro residents beyond those services funded by the City of San Leandro. Those <br /> additional Family Strengthening Program services are listed below and Girls Inc. will report out on all services in the narrative section of <br /> the quarterly reports. <br /> 1. Family crisis intervention (in conjunction with the Alameda County Crisis Receiving Home, Malabar House). <br /> 2. Respite care intervention for pre - delinquent and delinquent youth. <br /> 3. Family Communication Workshops. <br /> Mental Health Clinicians conduct family functioning matrices as well as child behavioral assessments. Program participants complete <br /> survey evaluations regarding quality of crisis intervention and /or respite intervention services. <br />
