PERFORMANCE MEASURES <br /> SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT <br /> 1. Provide individual and family therapy services and /or consultations services for a total of twenty (20) San Leandro children <br /> and families within a twelve (12) month period. <br /> 2. Assist seventy -five percent (75 %) or 15 of the 20 participating children and families to address psychological and <br /> communication problems within the family in order to alleviate parental abuse and /or child acting -out behavior. <br /> TIMELINES <br /> By October, 1, 2010, Gids•Inc. will have: <br /> • Served five (5) participating children and families with individual therapy, family therapy, and /or consultation services. <br /> • Demonstrated that seventy-five percent (75 %) or four (4) of the five (5) participating children and family members will be able to <br /> address psychological and communication problems within their family in order to alleviate parental abuse and /or child acting -out <br /> behavior. <br /> By January 1, 2011, Gins Inc. will have: <br /> • Served ten (10) participating children and families with individual therapy, family therapy, and /or consultation services. <br /> • Demonstrated that seventy-five percent (75 %) or eight (8) of the ten (10) participating children and family members will be able <br /> to address psychological and communication problems within their family in order to alleviate parental abuse and /or child acting - <br /> out behavior. <br /> By April 1, 2011, Girls Inc. will have accomplished: <br /> • Served fifteen (15) participating children and families with individual therapy, family therapy, and /or consultation services. <br /> • Demonstrated that seventy-five percent (75 %) or twelve (12) of the fifteen (15) participating children and family members will be <br /> able to address psychological and communication problems within their family in order to alleviate parental abuse and /or child <br /> acting -out behavior. <br /> By July 1, 2011, Gins Inc. will have accomplished: <br /> • Served twenty (20) participating children and families with individual therapy, family therapy, and /or consultation services. <br /> • Demonstrated that seventy -five percent (75 %) or sixteen (16) of the twenty (20) participating children and family members will be <br /> able to address psychological and communication problems within their family in order to alleviate parental abuse and /or child <br /> acting -out behavior. <br /> REPORTS AND MISCELLANEOUS <br /> 1. Agehcy must submit the City- provided Quarterly Reports within 15 days of the end of the each quarter. The Quarterly Reports <br /> must reflect the number of persons served, and must also include a narrative describing the services provided and results. The reports <br /> must be submitted even if there are no specific numbers to report, and the narrative report should describe why no specific numbers <br /> are reported. <br /> 2. To ensure timely expenditure of HUD funds, Agency must submit monthly reimbursement claims using the City's invoice or <br /> comparable form with documentation of monies claimed, including copies of time sheets (denoting specific hours for CDBG work <br /> performed), payroll stubs, DE3s or employment or contract letters for personnel costs, invoices or billings. <br /> 3. Housing Services Division staff will conduct CDBG desk monitoring on an ongoing basis based upon the City's CDBG <br /> monitoring plan. <br /> 4. Human Services staff and members of the Human Services Commission (HSC) and Housing Services Division staff will <br /> conduct at least one monitoring site visit per year. <br />
