The CONTRACTOR will collect and maintain all operating and performance data required by the CITY. All <br />reports and/or background data will be certified by the CONTRACTOR to be accurate. <br />The CONTRACTOR shall collect and maintain completed daily driver manifests and dispatch logs. These <br />will provide the base data for the monthly and annual performance and operating reports, fare box <br />reconciliation, and monthly CONTRACTOR billings. <br />The CONTRACTOR will maintain a set of completed, readable driver manifests and dispatch logs for a <br />period of three (3) years, and upon request be made available to the CITY for planning and auditing <br />purposes. <br />CONTRACTOR shall submit monthly reports to the CITY. CONTRACTOR shall have the ability to transmit <br />statistical reports via e-mail in a format that allows CITY to manipulate data as needed for reporting and <br />operational purposes. CONTRACTOR will be asked to collect and report data in format as required by <br />ACTIA and may be asked to collect further data as required by ACTIA. Such reporting may include <br />financial records related to the delivery of services, and operating service statistics. The CONTRACTOR <br />must be prepared to provide operating statistics that include, but are not limited to, the following applicable <br />data sought by ACTIA: <br />1. Individual trips provided to ambulatory passengers <br />2. Individual trips provided to non - ambulatory passengers <br />3. Same -day (demand- response) trips provided. <br />4. Prescheduled and subscription trips provided. <br />5. Group trips provided. <br />6. Other specified trips provided. <br />7. Attendants riding along on trips provided. <br />8. Companions riding along on trips provided. <br />9. Trips provided on behalf of other paratransit programs by the CITY. <br />10. Vehicle revenue hours for trips provided under this AGREEMENT. <br />11. Vehicle miles for trips provided under this AGREEMENT. <br />12. Number and percent of trip requests canceled by passengers. <br />13. Number and percent of trip requests denied to passengers. <br />14. Number and percent of passenger No- Shows. <br />15. Number and percent of passenger "disputed" No- Shows. <br />16. Number and percent of contractor No- Shows. <br />17. Percent of early, late and on -time pick -ups. <br />18. Percent of early, late and on -time drop -offs. <br />19. Percent trips over the "ride- time" standard <br />20. Average ride -time <br />21. Number of vehicle /property damage occurrences only <br />22. Number of personal injury occurrences only <br />23. Number of vehicle /property damage and personal injury occurrences. <br />24. Total accidents and incidents. <br />25.
