TIMELINES <br />By October, 1, 2011, Davis Street will have accomplished: <br />• Provide 3,102 families with basic needs services to include any of the above mentioned services <br />By January 1, 2012, Davis Street will have accomplished: <br />Provide 6,205 families with basic needs services to include any of the above mentioned services <br />including case management to families that volunteer to monitor and track progress. <br />By April 1, 2012, Davis Street will have accomplished: <br />Provide 9,307 families with basic needs services to include any of the above mentioned services <br />including case management to monitor and track progress. <br />By July 1, 2012, Davis Street will have accomplished: <br />• Provide 12,408 families with basic needs services to include any of the above mentioned services <br />including case management to monitor and track progress. <br />• Approximately forty percent (40 %) or 4,964 families, who accessed the basic needs services will <br />demonstrate increased financial or housing stability as indicated on their intake assessment and follow <br />up at the 4 th and 12 month re- assessment periods. Families will become more self- sufficient by <br />having maintained their housing, as reported at time of follow up. <br />• Approximately forty percent (40 %) or 4,964 families will report a decrease in the risk of compromising <br />their self- sufficiency by having maintained employment, as reported at time of follow -up. <br />• Approximately forty percent (40 %) or 4,964 under - employed families who receive comprehensive <br />support services will reduce the risk of losing their jobs by maintaining employment during the first <br />month following their crisis or use of services <br />-14- <br />CDBG Agreement with DSFRC — <br />Basic Needs Program FY11 -12 <br />
