PERFORMANCE MEASURES <br /> SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT <br /> 1. Provide individual and family therapy services and /or consultations services for fifteen (15) San <br /> Leandro children and family members served within twelve (12) months. <br /> 2. Assist seventy -five percent (75 %) or 11 of the 15 participating children and family members to address <br /> psychological and communication problems within their family in order to alleviate parental abuse <br /> and /or child acting -out behavior. <br /> TIMELINES <br /> By October, 1, 2011, Girls Inc. will have: <br /> • Served four (4) participating children and family members with individual therapy, family therapy, and /or <br /> consultation services. <br /> • Demonstrated that seventy -five percent (75 %) or three (3) of the four (4) participating children and <br /> family members will be able to address psychological and communication problems within their family <br /> in order to alleviate parental abuse and /or child acting -out behavior. <br /> By January 1, 2012, Girls Inc. will have: <br /> • Served eight (8) participating children and family members with individual therapy, family therapy, <br /> and /or consultation services. <br /> • Demonstrated that seventy -five percent (75 %) or six (6) of the eight (8) participating children and family <br /> members will be able to address psychological and communication problems within their family in order <br /> to alleviate parental abuse and/or child acting -out behavior. <br /> By April 1, 2012, Girls Inc. will have accomplished: <br /> • Served twelve (12) participating children and family members with individual therapy, family therapy, <br /> and /or consultation services. <br /> • Demonstrated that seventy -five percent (75 %) or nine (9) of the twelve (12) participating children and <br /> family members will be able to address psychological and communication problems within their family <br /> in order to alleviate parental abuse and /or child acting -out behavior-: - <br /> By July 1, 2012, Girls Inc. will have accomplished: <br /> • Served fifteen (15) participating children and family members with individual therapy, family therapy, <br /> and /or consultation services. <br /> • Demonstrated that seventy -five percent (75 %) or eleven (11) of the fifteen (15) participating children <br /> and family members will be able to address psychological and communication problems within their <br /> family in order to alleviate parental abuse and /or child acting -out behavior. <br /> - 14 - <br /> CDBG Agreement with Girls Inc.
