EXHIBIT A SCOPE OF WORK Term of Contract: July 1, 2011 — June 30, 2012 Agency: Safe Alternatives to Violent Environments (SAVE) Address: 1900 Mowry Avenue, Suite 204, Fremont, CA 94538 Contact Person: Rodney Clark, Executive Director Phone: 510- 574 -2250 Project Name: SAVE — Community Oriented Prevention Services (COPS) Program PROJECT GOALS AND OBJECTIVES SAVE's Community Oriented Prevention Services (COPS) Program is partnership between SAVE and the police departments of San Leandro, Hayward, Fremont, and Union City working to prevent and reduce domestic violence, and to provide services to victims and their children. SAVE promotes alternatives to domestic violence through support services, advocacy, and education. SAVE also helps domestic violence victims and their families end the cycle of violence by providing crisis intervention services that include crisis counseling, domestic violence education, case management, and restraining order assistance. MAJOR ACTIVITIES SAVE will provide Teen Dating Violence Presentations to middle and high schools throughout the Alameda County. The presentations provide middle and high school students with information about domestic violence, including the dynamics of healthy vs. unhealthy intimate relationships, thus empowering them with knowledge that promote their well -being and actively striving to end the cycle of violence. SAVE will provide crisis counseling, domestic violence education, case management, and restraining order assistance for victims of domestic violence. Receiving these crisis intervention services early in the cycle of violence allow victims to increase their level of safety and self - sufficiency. — PERFORMANCE MEASURES SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT • 1. Have 300 students from middle and high schools in San Leandro and surrounding communities participate in a Teen Dating Violence Prevention Presentation. 2. Increase the knowledge of domestic violence and the resources available to domestic violence victims to seventy -five percent (75 %) (225 of 300) of the students that participates in the Teen Dating Violence Prevention Presentations. 3. Provide crisis intervention services, domestic violence education, and /or referrals to 150 Alameda County residents who are domestic violence victims and have contact with law enforcement. 4. Increase the level of self- sufficiency of seventy -five percent (75 %) (112 of 150) of the Alameda County residents, who are victims and have made domestic violence reports, by providing them crisis intervention services and domestic violence education which teach them new safety skills. - 13 - CDBG Agreement with SAVE FY11 -12
