CONDITIONS OF APPROVAL <br /> RECOMMENDED <br /> CONDITIONS OF APPROVAL <br /> PLN2011 -00023 — 1300 Bancroft Avenue #201 <br /> D. Nguyen (Applicant) and B. Warhurst (Property Owner) <br /> COMPLIANCE WITH APPROVED PLANS <br /> A. The property owner shall comply with Exhibits A, dated October 6, 2011, except as <br /> hereinafter modified. (Copies of Exhibits are on file in the Community Development <br /> Department, 835 East 14th Street, San Leandro, California). <br /> Exhibit A — Floor Plan <br /> 13. The applicant and/or property owner shall be responsible for assuring that any successor in <br /> interest who assumes responsibility for this conditional use permit approval is informed of <br /> its terms and conditions. <br /> 11. PERMIT!' ED USE <br /> A. This modification is to allow for the addition of three (3) massage tables within three <br /> (3) semi- private rooms, in addition to the chair massage services previously permitted; <br /> 1300 Bancroft Avenue, Suite 201; Alameda County Assessor's Parcel Number 77 -524- <br /> 12 -4. <br /> 13. This conditional use permit approval modifies Conditional Use Permit PLN2008- <br /> 00042, and retains all Conditions of Approval of that approval with the new conditions <br /> noted in bold. <br /> C. This use permit allows for a total of no more than six (6) massage stations; three <br /> massage chairs in one open space, and three tables within three semi - private rooms. <br /> Use of additional space in this building for massage therapy, whether it is an <br /> expansion of this business or a new massage therapy business, shall be required to go <br /> through a separate Conditional Use Permit process. <br /> D. Two of the semi - private rooms for table massage shall be created with cubicle -style <br /> walls not to exceed six feet in height, and the doors shall not have locks. An existing <br /> room shall be used for the third table massage space and shall have no door, but a <br /> curtain instead. The three rooms shall be located as noted on Exhibit A. <br /> I . The business shall not open prior to 9:30 a.m. and shall not receive or schedule any <br /> clients after 6:00 p.m. with the business closing no later than 7:15 p.m., Monday to <br /> Friday. Saturday hours shall have the same 9:30 a.m. starting time and the last received <br /> client by no later than 3:45 p.m. and the business must close by no later than 5 p.m. <br /> The business is not permitted to open on Sundays. <br /> Agreement to Conditions October 6, 2011 <br /> PLN20I 1 -00023 Page 3 of 4 <br />
