• F. All employees of this business or contractors with the business involved in massage of <br /> clients shall be Certified Massage Therapists (CMTs) and shall obtain individual massage <br /> therapist permits in compliance with Municipal Code Section 4 -23 -400. <br /> G. All clients shall be seen at the business location at 1300 Bancroft Avenue; out -call <br /> massage visits are prohibited by Municipal Code Section 4- 23 -540. <br /> 11. Any complaints regarding this business may be grounds for a review hearing by the Board <br /> of Zoning Adjustments or revocation of the business license, subject to a decision by the <br /> Community Development Director and the Police Chief. <br /> 111. GENERAL REQUi REM ENl'S <br /> A. For the purpose of determining compliance with Uniform Safety Codes, plans must be <br /> submitted to Building Safety Services and a building permit obtained prior to installation <br /> of the cubical walls and doors for the two new semi- private rooms. <br /> 13. Prior to use of the two new semi- private rooms, all applicable Fire and Building Code <br /> requirements shall be completed to the satisfaction of the Alameda County Fire Marshall <br /> and the City of San Leandro Chief Building Official, and the building permit finalized. <br /> C. The project site shall be well maintained and shall be kept free of litter and debris at all <br /> times. <br /> D. Any graffiti shall be promptly removed from building walls, windows or doors. <br /> E. The approvals granted by the City as a result of this application, as well as the Conditions <br /> of Approval, shall be recorded in the office of the County Recorder of Alameda County. <br /> F. No application for amendment of the application or Conditions of Approval may be <br /> submitted or accepted for processing by the City unless (i) there is full compliance with all <br /> terms of the application and Conditions of Approval; or (ii) the Development Services <br /> Director waives compliance with the terms of the application and Conditions of Approval <br /> pending application for amendment: <br /> G. Pursuant to Zoning Code Section -2218, this approval shall lapse on October 6, 2012, <br /> unless a) a business license has been issued, coupled with diligent progress evidencing a <br /> good faith intent to commence the intended use, or b) a written request for a one -year <br /> extension of the use permit is approved by the Zoning Enforcement Official. <br /> Agreement to Conditions October 6, 2011 <br /> PLN201 1 -00023 Page 4 of 4 <br />
