TIMELINES <br />By October, 1, 2012, Davis Street will have accomplished: <br />• Provide 27 San Leandro clients with crisis intervention and /or short term community counseling <br />services and access to eligible family resource center services. <br />• Twenty five (25 %) or 2 of 96 San Leandro clients will report a reduction in the severity or frequency of <br />the presenting issue as supported by a decrease in high -risk behavior. <br />By January 1, 2013, Davis Street will have accomplished: <br />• Provide 54 San Leandro clients with crisis intervention and /or short term community counseling <br />services and access to eligible family resource center services. <br />• Fifty (50 %) or 27 of the 96 San Leandro clients will report a reduction in the severity or frequency of the <br />presenting issue as supported by a decrease in high -risk behavior. <br />By April 1, 2013, Davis Street will have accomplished: <br />• Provide 81 San Leandro clients with crisis intervention and /or short term community counseling <br />services and access to eligible family resource center services. <br />• Sixty percent (60 %) or 57 of the 96 San Leandro clients will report a reduction in the severity or <br />frequency of the presenting issue as supported by a decrease in high -risk behavior. <br />By July 1, 2013, Davis Street will have accomplished: <br />• Provide 96 San Leandro clients with crisis intervention and /or short term community counseling <br />services and access to eligible family resource center services. <br />• Seventy -five percent (75 %) or 72 of the 96 San Leandro clients will report a reduction in the severity or <br />frequency of the presenting issue as supported by a decrease in high -risk behavior. <br />-14- <br />CDBG Agreement with DSFRC — <br />Community Counseling Program FY12 -13 <br />
