TIMELINES By October, 1, 2012, Davis Street will have accomplished: • Provide 27 San Leandro clients with crisis intervention and /or short term community counseling services and access to eligible family resource center services. • Twenty five (25 %) or 2 of 96 San Leandro clients will report a reduction in the severity or frequency of the presenting issue as supported by a decrease in high -risk behavior. By January 1, 2013, Davis Street will have accomplished: • Provide 54 San Leandro clients with crisis intervention and /or short term community counseling services and access to eligible family resource center services. • Fifty (50 %) or 27 of the 96 San Leandro clients will report a reduction in the severity or frequency of the presenting issue as supported by a decrease in high -risk behavior. By April 1, 2013, Davis Street will have accomplished: • Provide 81 San Leandro clients with crisis intervention and /or short term community counseling services and access to eligible family resource center services. • Sixty percent (60 %) or 57 of the 96 San Leandro clients will report a reduction in the severity or frequency of the presenting issue as supported by a decrease in high -risk behavior. By July 1, 2013, Davis Street will have accomplished: • Provide 96 San Leandro clients with crisis intervention and /or short term community counseling services and access to eligible family resource center services. • Seventy -five percent (75 %) or 72 of the 96 San Leandro clients will report a reduction in the severity or frequency of the presenting issue as supported by a decrease in high -risk behavior. -14- CDBG Agreement with DSFRC — Community Counseling Program FY12 -13
