EXHIBIT A SCOPE OF WORK Term of Contract: July 1, 2012 — June 30, 2013 Agency: Girls Incorporated of Alameda County Address: 13666 East 14" Street San Leandro, CA 94706 Contact Person: Rebecca Cannon, Chief Mental Health Officer Phone: 510 - 357 -5515 Project Name: Pathways Counseling Center's Family Strengthening Program PROJECT GOALS AND OBJECTIVES The Pathways Counseling Center's Family Strengthening Program will provide individual and family therapy services and Parent /Family Workshops that enable individuals and families to address psychological and communication problems within their families thus increasing the likelihood that these problems do not escalate into parental abuse or adolescent acting -out behavior. The Family Strengthening Program will also include sharing community resources to help families to access health and other family - related services. The program will also provide family crisis intervention services, in which children and families can engage in order to develop communication skills and psychological capacity to overcome the current "hot" crisis of their families and reunite as a family, and respite care intervention for pre - delinquent and delinquent youth. Services will be delivered in Spanish and English. MAJOR ACTIVITIES Pathways services are: Individual and family therapy services as well as consultation services with other professionals involved in the cases (for example, teachers, school administrators, pediatricians, psychiatrists, etc.) Mental Health Clinicians will conduct clinical assessments (measured on a 10 -point scale conducted at: intake, subsequent 90 -day intervals, and termination). Participating family members will complete communication skills assessments as well as survey evaluations regarding quality of treatment services. Many additional services are delivered for San Leandro residents beyond those services funded by the City of San Leandro. Those additional Family Strengthening Program services are listed below. Girls Inc. will provide a summary of these services in the narrative section of the quarterly reports. 1. Family crisis intervention (in conjunction with the Alameda County Crisis Receiving Home, Malabar House). 2. Respite care intervention for pre - delinquent and delinquent youth. 3. Effective Parenting Workshops Mental Health Clinicians conduct family functioning matrices as well as child behavioral assessments. Program participants complete survey evaluations regarding quality of crisis intervention and /or respite intervention services. -13- CDBG Agreement with Girls Inc. FY12 -13
