VI. REPORTING AND MISCELLANEOUS <br /> Consultant shall submit the City - provided Quarterly Reports within fifteen (15) days of the end of <br /> each quarter. The quarterly and narrative reports shall include, but not be limited to, the following <br /> data elements: <br /> A. Name and address of applicants <br /> B. Date of application <br /> C. Date of site visit <br /> D. Dates of start and completion <br /> E. Description of work performed • <br /> F. Costs of work performed <br /> G. Name of contractor <br /> H. Income and ethnicity of clients <br /> I. Total number of applications <br /> J. Total number of units under construction <br /> K. Total number of units completed or committed <br /> L.
