Under no circumstances will coverage be allowed <br /> retroactively. <br /> Those whose health plan coverage was allowed to lapse for a duration greater <br /> than three months will be able to re- enroll within thirty calendar days of the <br /> date they return to work in the same manner as is allowed for new hires; <br /> however, employees re- enrolling in Blue Cross will be subject to medical review - <br /> to determine evidence of good health and effective date of coverage will be <br /> determined by Blue Cross after all medical information is received and <br /> reviewed. Such employees will be subject to new deductibles, maximums, and <br /> waiting periods. <br /> 7. 30 -DAY RE- ENROLLMENT: For employees who are enrolled in the Spousal <br /> plan, an employee whose spouse's health plan coverage is no longer available, <br /> may, within thirty calendar days of such loss of coverage, enroll in one of the <br /> County/Department's Blue Cross Health Plans. <br /> 8. OPEN ENROLLMENT: Eligible employees may choose from among Kaiser, <br /> Health Net, and the Blue Cross options dining a special Open Enrollment <br /> • period in the Fall of 1995 and annually thereafter. <br /> 9. PERS.HEALTH BENEFITS PROGRAM - Instead of the foregoing options <br /> provided under subsection 13.A.1.a. of this Section, the represented employees <br /> may choose to participate in the Public Employees' Retirement System's <br /> (PERS) Health Benefits Program. Employees not currently enrolled in the <br /> PERS Health Benefits Program may select this option as soon as possible <br /> following approval by the Public Employees' Retirement System. Once an <br /> employee participates in the PERS Health Benefits Program, he or she may <br /> not return to the County Health Benefits Program. <br /> a. The parties to this agreement understand that any employee who <br /> chooses to participate must abide by the eligibility requirements and <br /> deadlines as set forth in the PERS guidelines for participation in their <br /> Health Benefits Program. <br /> h. For Active Employees: The Department shall contribute up to a <br /> maximum as established by the Public Employee Retirement System's <br /> Kaiser North single -party premium with respect to enrollment of an <br /> employee only; up to Kaiser North two -party premium with respect to <br /> enrollment for an employee and one dependent: and up to Kaiser North <br /> three -party (family) premium with respect to enrollment for an <br /> employee and two or more dependents for the benefit coverage offered <br /> by PERS. <br /> c. For Retirees: Department employees who have already retired or who <br /> thereafter retire, under the Public Employees' Retirement. System and <br /> PERS' Irealth Benefits Program, will be entitled to a Department <br /> contribution for retirees up to a maximum of $171.S1 per month for the <br /> 1994 -95 fiscal year.
