HEALTH AND DENTAL PLANS (Contd.) <br /> monthly providers charge for a comprehensive group health plan by a health <br /> maintenance organization or toward an indemnity health plan for eligible <br /> full -time employees as well as their spouses and eligible dependents up to <br /> $156.54 per month for an employee with no dependents; up to $313.08 per <br /> month for an employee with one dependent; up to $416.40 per month for an <br /> employee with two or more dependents. <br /> 3. For coverage from February 1, 1996 through the remaining term of this <br /> Memorandum of Understanding, the Department shall contribute toward the <br /> • monthly provider's charge for a comprehensive group health plan by a health • <br /> maintenance organization or toward one of the indemnity options for eligible <br /> full -time employees, as well as their spouses and eligible dependents, up to but <br /> • not exceeding the actual monthly charges established by the Kaiser Health <br /> Foundation for the same benefit coverage provided in subsection 13.A.1. hereof. <br /> In no event shall the Department contribution exceed the premiuun of the <br /> options selected. <br /> 4. The Department contribution toward the providers charge shall be the <br /> full -time contribution provided that the employee is on full -time paid status. <br /> If the employee is on paid status on less than a full -time basis, the Department <br /> contribution shall be the full -time contribution prorated each pay period based <br /> on the proportion of the hours on paid status within that pay period to the <br /> normal full time hours for the job classification, provided further that the <br /> employee is on paid status at least 50 percent of the normal full -time pay <br /> period for the job classification. <br /> 5. DUPLICATIVE COVERAGE: Applies to married County/Department <br /> employees, both employed by the County/Department. The intent of this <br /> section limits married County/Department employees from both covering each <br /> other within the same health plan. Married County/Department employees, <br /> both employed by the County/Department, shall be entitled to one choice from <br /> the following list of health plan coverages: <br /> a. Up to one full family Kaiser membership. <br /> b. Up to one full family Blue Cross membership. • <br /> c. Up to one full family Health Net membership. <br /> d. Up to one full family Blue Cross membership (excluding California <br /> Care) together with up to one Blue Cross spousal option. <br /> e. Up to one full family Kaiser or Health Net membership together with <br /> up to one full family Blue Cross membership. <br /> • f. Up to one full family Kaiser membership with up to one full fancily <br /> Health Net membership. <br /> 6. EFFECT OF AUTHORIZED LEAVE WITHOUT PAY ON HEALTH PLAN <br /> COVERAGE: Employees who were absent on authorized leave without pay, <br /> and whose health plan coverage was allowed to lapse for a duration of three <br /> mouths or less, will be able to re -enroll as a continuing member in the same <br /> plan under which they had coverage prior to the authorized leave by <br /> completing the appropriate enrollment cards within thirty calendar days of the <br /> date they return to work.
