For coverage from February 1, 1996 through the remaining term of this <br /> Memorandum of Understanding, the Department shall contribute toward the <br /> monthly provider's charge for a comprehensive group health plan by a health <br /> maintenance organization or toward one of the indemnity options for eligible <br /> full-time employees, as well as their spouses and eligible dependents, up to but not <br /> exceeding the actual monthly charges established by the Kaiser Health Foundation <br /> for the same benefit coverage provided in subsection 13A1. hereof. In no event <br /> • shall the Department contribution exceed the premium of the options selected. <br /> 4. The Department contribution toward the providers charge shall be the full-time <br /> contribution provided that the employee is on full-time paid status. If the employee <br /> is on paid status on less than a full-time basis, the Department contribution shall <br /> be the full-time contribution prorated each pay period basedon the proportion of the <br /> hours on paid status within that pay period to the normal full time hours for the <br /> job classification, provided further that the employee is on paid status at least 50 <br /> percent of the normal full-time pay period for the job classification. <br /> 5. DUPLICATIVE COVERAGE: Applies to married County/Department employees, <br /> both employed by the County/Department. The intent of this section limits married <br /> County/Department employees from both covering each other within the same <br /> health plan. Married County/Department employees, both employed by the <br /> County/Department, shall be entitled to one choice from the following list of health <br /> plan coverages: <br /> a. Up to one full family Kaiser membership. <br /> b. Up to one full family Blue Cross membership. <br /> • <br /> c. Up to one full family Health Net membership. <br /> d. Up to one full family Blue Cross membership (excluding California Care) <br /> together with up to one Blue Cross spousal membership. <br /> e. Up to one full family Kaiser or Health Net membership together with up to <br /> one full family Blue Cross membership. <br /> f. Up to one full family Kaiser membership with up to one full family Health <br /> Net membership. <br /> 6. EFFECT OF AUTHORIZED LEAVE WITHOUT PAY ON HEALTH PLAN <br /> COVERAGE: Employees who were absent on authorized leave without pay, and <br /> whose health plan coverage was allowed to lapse for a duration of three months or <br /> less, will be able to re -enroll as a continuing member in the same plan under which <br /> they had coverage prior to the authorized leave by completing the appropriate <br /> enrollment cards within thirty calendar days of the date they return to work. The <br /> deductibles, maximums, and waiting periods shall be applied as though the <br /> employee had been continuously enrolled. Coverage will begin on the first of the <br /> month following the month in which re- employment was effective.
