Effective January 1, 1995, there will be a general salary increase for represented <br /> classifications of three percent above the then existing levels, as specifically set forth in <br /> Appendix A. <br /> C. EMERGENCY MEDICAL TECHNICIAN. Eligible employees shall be compensated an <br /> additional 2 -1/2% of the base salary as specified in Appendix A, for obtaining and <br /> maintaining EMT -1 and EMT -D certificates. <br /> SECTION 11. HEALTH AND DENTAL PLANS <br /> • <br /> A. HEALTH PLAN COVERAGE • - <br /> • <br /> 1. For coverage from January 1, 1994 through January 31, 1995, the County/District <br /> shall contribute towards the monthly provider's charge for a comprehensive group <br /> health plan by a health maintenance organization or toward an indemnity health <br /> plan for eligible full-time employees, as well as their spouses and eligible <br /> dependents up to $173.04 per month for an employee with no dependents; up to <br /> $346.08 per month for an employee with one dependent; up to $460.28 per month <br /> for an employee with two or more dependents. This contribution shall apply to the <br /> health plan options listed below. <br /> a. Indemnity Options: <br /> (1) Blue Cross Prudent Buyer (identified as H -2 in the employee <br /> handbook) <br /> • <br /> (2) Spousal Coverage Plan (identified as H -3 in the employee handbook) <br /> b. Kaiser (identified as H -4 in the employee handbook) <br /> c. California Care (identified as H -5 in the employee handbook). <br /> d. Health Net - Plan T (identified as H -6 in the employee handbook). <br /> 2.
