- 13 - <br />CDBG Agreement with Spectrum Community Services FY13-14 <br />EXHIBIT A <br /> <br />SCOPE OF WORK <br /> <br />Term of Contract: July 1, 2013 – June 30, 2014 <br /> <br />Agency: Spectrum Community Services <br /> <br />Address: 2621 Barrington Court <br /> Hayward, CA 94545 <br /> <br />Contact Person: Michael Sweeney, Executive Director <br /> <br />Phone: 510-881-0300 <br /> <br />Project Name: Senior Nutrition And Activities Program (SNAP) – Senior Lunch Program <br /> <br /> <br />PROJECT GOALS AND OBJECTIVES <br /> <br />Based on the funds granted, Spectrum Community Services will serve 322 San Leandro seniors--7,370 hot, <br />nutritious meals per year at San Leandro Senior Community Center and Fargo Senior Apartments. <br /> <br /> <br />MAJOR ACTIVITIES <br /> <br />SNAP meals will be served five days a week, 248 days per year, at the San Leandro Senior Community Center <br />and the Fargo Senior Center. Each site schedule is customized to the needs of the sponsoring facility and the <br />senior clients. Each day, hot, nutritious lunch-time meals will be prepared at Spectrum’s central kitchen <br />located at Josephine Lum Lodge in Hayward. The meals are carefully planned and prepared to be delicious, <br />low salt, low fat, and nutritionally balanced to supply 1/3rd of the U.S. Recommended Daily Allowance for older <br />adults. After preparation, meals will be packed in bulk containers and delivered by Spectrum’s Van Driver to <br />the two San Leandro dining sites. <br /> <br />A Spectrum Site Coordinator will work with SNAP staff, facility staff and volunteers at each site to serve the <br />meals. The SNAP staff or volunteers welcome each participant for lunch, log participants and register new <br />participants, and receive voluntary donations. No senior is ever turned away for lack of money. To protect <br />participants from the embarrassment of receiving “charity”, Spectrum will maintain records of donation amounts <br />received at each site and will apply the subsidy to cover the difference between actual donation and the $3.25 <br />suggested voluntary donation. <br /> <br />As part of Spectrum’s SNAP program, SNAP staff make regular presentations on healthy living and nutrition <br />and take questions from program participants. We include nutrition information and tips on the backs of SNAP <br />menus, and SNAP staff and volunteers are trained to reinforce connections between nutrition, physical activity <br />and health. <br />
