CityofSanLeandroAnnualActionPlan <br />2014 <br />18 <br />OMBControlNo:2506Ͳ0117(exp.07/31/2015) <br />APͲ65HomelessandOtherSpecialNeedsActivities–91.220(i) <br /> <br />Introduction <br />EveryOneHomeactivitiestoaddressthehousingneedsofthehomelessandextremelylowͲincome <br />personswithseriousmentalillnessand/orthoselivingwithHIV/AIDScenterontheimplementationof <br />theEveryOneHomePlan.Planningeffortsbeganin2004asauniquecollaborationamongcommunity <br />stakeholders,cities,andAlamedaCountygovernmentagenciesrepresentingthree(3)separatecare <br />systems—homelessservices,HIV/AIDSservices,andmentalhealthservices—thatshareoverlapping <br />clientpopulationsandarecognitionthatstablehousingisacriticalcornerstonetothehealthandwellͲ <br />beingofhomelessandatͲriskpeople,andourcommunities.Sinceitspublicationin2006,thePlanhas <br />beenadoptedbytheAlamedaCountyBoardofSupervisorsandallfourteen(14)cities,andendorsedby <br />numerouscommunityorganizations. <br /> <br />ImplementationoftheEveryOneHomePlanisbeingspearheadedbyacommunityͲbasedorganization <br />ofthesamename.ItisguidedbyaLeadershipBoardcomprisedofjurisdictionalappointedmembers <br />andkeycommunityconstituenciessuchasconsumers,cities,nonprofits,businesses,andfaithͲbased <br />organizations. <br /> <br />EveryOneHomeenvisionsasystemofhousingandservicesinAlamedaCountythat,by2020,ensuresall <br />extremelylowͲincomeresidentshaveasafe,supportive,andpermanentplacetocallhomewith <br />servicesavailabletohelpthemstayhousedandimprovethequalityoftheirlives.EveryOneHome <br />partnersareworkingonfive(5)corestrategies: <br />1.Preventhomelessnessandotherhousingcrises. <br />2.IncreasehousingopportunitiesforhomelessandextremelylowͲincomehouseholds. <br />3.Deliverflexibleservicestosupportstabilityandindependence. <br />4.Measuresuccessandreportoutcomes. <br />5.DeveloplongͲtermleadershipandbuildpoliticalwill. <br /> <br />DescribethejurisdictionsoneͲyeargoalsandactionsforreducingandendinghomelessness <br />including <br /> <br />Reachingouttohomelesspersons(especiallyunshelteredpersons)andassessingtheir <br />individualneeds <br />x Continuingtocoordinatetheimplementationofcountywideapproachestorapidrehousingand <br />homelessnessprevention,includinguseofEmergencySolutionsGrantfundsalongwith <br />resourcesfromTANF,FosterCareandProbation.Workonadditionalopportunitiestoexpand <br />RapidRehousingresourcesthroughouttheCounty.Theintentionistobuildonthebest <br />practicesofthePriorityHomePartnership,AlamedaCounty’sHomelessPreventionandRapid <br />RehousingprogramfundedbyARRA,andmakingadjustmentsfromthelessonslearnedandthe <br />reductionsinresourcespoststimulus. <br /> <br />x HomelessPointͲInͲTimeCountReporttocommunityandjurisdictions. <br /> <br />x Continuingtoworktoexpandhousingopportunities.Thejurisdictionalcommitteewillworkto <br />ensurethateachcommunityiscontributingtothecreationofpermanenthousingaffordable <br />andaccessibletotheEveryOneHometargetpopulations.Collaborationwillcontinuewithlocal
