CityofSanLeandroAnnualActionPlan <br />2014 <br />19 <br />OMBControlNo:2506Ͳ0117(exp.07/31/2015) <br />housingauthoritiestoincreasevouchersavailabletothepopulationaswell.Publishingthe <br />secondannualEveryOneHomeUnitProgressReportinthespringof2013. <br /> <br />x InsupportoftheperformanceoutcomesinitiativeEveryOneHomewillcollaboratewiththe <br />HMISteamtoproducetheannualOutcomesProgressReportinthespringof2013reportingon <br />performancefor2012calendaryear.Continuetoprovidetechnicalassistanceforprovider <br />agenciesincludingtheEveryOneHomeHousedAcademyinMarch2013.ThetwoͲdayEveryOne <br />HousedAcademywillhelpagenciesdevelopingchangesintheiroperatingpoliciesand <br />proceduresthatwillenablethemtopermanentlyhousepeoplemorequicklyandperform <br />betterontheHEARTHActoutcomesaswellasourlocalcountyͲwideoutcomes. <br /> <br />x ContinuetoworkonthetwosystemͲwideevaluationandplanningprocessesstartedin2012for <br />movingtowardamorecentralizedcoordinatedintakeforhomeless/housingservicesandfor <br />maximizingthebenefitsofourtransitionalhousingprograms. <br /> <br />x Continuingtokeepthecommunityinformedthroughnewsletters,meetingsandwebsite,ofthe <br />collectiveeffortstopreventandendhomelessnessandtheprogressmadeonachievingthe <br />goals. <br /> <br />211isacentralaccesspointinAlamedaCountyforhomelesspersonsseekingshelterandotherservice <br />needs.Inaddition,theHopeProjectprovidesoutreachtostreethomelessinsouthandeastAlameda <br />County.HealthCarefortheHomelessprovideshealthcaretohomelessinshelterandonthestreet.All <br />oftheseprogramsprovideaninitialassessmenttodetermineindividualneedsandworkstomeetthose <br />needs,eitherdirectlyorthroughreferrals. <br /> <br />Addressingtheemergencyshelterandtransitionalhousingneedsofhomelesspersons <br />FESCOprovidesbothemergencyshelterandtransitionalhousinginmidͲcounty.AbodeServicesprovide <br />emergencyshelter,transitionalhousingandpermanenthousinginsouthandeastcounties.TheSHPͲ <br />fundedSouthernAlamedaCountyHousing/JobsLinkagesprogramprovidesrapidrehousingtosheltered <br />homelessinmid,south,andeastcounties.TheNorthCountyWinterShelter,jointlyfundedbytheCities <br />ofBerkeleyandOakland,provides100bedspernighttohomelessinnorthAlamedaCountyfrom <br />NovemberthroughApril.TheSouthCountyWinterReliefProgramusesrapidrehousingstrategiesto <br />house15familiesinsouthandeastcounties. <br /> <br />Helpinghomelesspersons(especiallychronicallyhomelessindividualsandfamilies,families <br />withchildren,veteransandtheirfamilies,andunaccompaniedyouth)makethetransitionto <br />permanenthousingandindependentliving,includingshorteningtheperiodoftimethat <br />individualsandfamiliesexperiencehomelessness,facilitatingaccessforhomelessindividuals <br />andfamiliestoaffordablehousingunits,andpreventingindividualsandfamilieswhowere <br />recentlyhomelessfrombecominghomelessagain <br />x AlamedaCountyHousingandCommunityDevelopmentDepartmentthroughHMISand <br />leadershipoftheEveryOneHomePerformanceManagementCommitteeissupportiveofthe <br />EveryOneHomeinitiativetoestablishsystemwideoutcomesandtoevaluateeffectivenessof <br />programsagainstthoseoutcomes.Theseoutcomesincludeshorteningtheperiodoftime <br />homelessandreducingtherecidivismratesforhomelesspeople.AnannualOutcomesReportis
