EXHIBIT A <br /> <br />SCOPE OF WORK <br /> <br />Term of Contract: July 1, 2015 – June 30, 2016 <br /> <br />Agency: Spectrum Community Services <br /> <br />Address: 2621 Barrington Court <br /> Hayward, CA 94545 <br /> <br />Contact Person: Lara Calvert, Executive Director <br /> <br />Phone: 510-881-0300 <br /> <br />Project Name: Senior Meal Program <br /> <br /> <br />PROJECT GOALS AND OBJECTIVES <br /> <br />Based on the funds granted, Spectrum Community Services will serve 450 San Leandro seniors—7,900 hot, <br />nutritious meals per year at San Leandro Senior Center and Fargo Senior Apartments. <br /> <br /> <br />MAJOR ACTIVITIES <br /> <br />Senior Meal Program lunches will be served five (5) days a week, 248 days per year, at the San Leandro <br />Senior Community Center, the Fargo Senior Center, and Ashland Community Center. Each site schedule is <br />customized to the needs of the sponsoring facility and the senior clients. Each day, hot, nutritious lunch-time <br />meals will be prepared at Spectrum Community Services’ commercial kitchen located at Josephine Lum Lodge <br />in Hayward. The meals are carefully planned and prepared to be delicious, low salt, and nutritionally balanced <br />to supply 1/3rd of the U.S. Recommended Daily Allowance for older adults. After preparation, meals will be <br />packed in bulk containers and delivered by Spectrum’s Van Driver to the two San Leandro dining sites. <br /> <br />A Spectrum Site Coordinator will work with the Senior Meal Program staff, facility staff, and volunteers at each <br />site to portion the food and serve the meals restaurant style to the seniors. The Senior Meal Program staff or <br />volunteers welcome each participant for lunch, log participants, register new participants, and receive voluntary <br />donations. No senior is ever turned away for lack of money. To keep participants’ voluntary donations <br />confidential, Spectrum will maintain records of donation amounts received at each site, not by each participant. <br />Our fundraising is done to help cover the costs for seniors who cannot give the suggested $3.75 donation. <br /> <br />As part of the Senior Meal Program, Spectrum staff make regular nutrition presentations on healthy living and <br />eating and take questions from program participants. We include nutrition information and tips on the backs of <br />Senior Meal Program menus, and staff and volunteers are trained to reinforce connections between nutrition, <br />physical activity, and health. <br />
