Pursuant to the provisions set forth in paragraph III above, Recipient agrees to make reports to HUD-FHEO, addressed in accordance with paragraph IV.A above,regarding its actions to comply with the terms of this Agreement, at the following intervals following execution of this Agreement: Due 120 days from date of execution: III.A—Copy of policy to include TDD/CRS number on CDBG-related documents and notices; III.B—Copies of Section 504 Coordinator appointment letter(s); Due 180 days from date of execution: IILF—Report on actions taken to analyze and identify need for affirmative marketing to potentially under-served groups in funded programs and activities; Within 210 days from date of execution: III.0—Copy of Section 504 Grievance Policy, date of training to staff on policy, copy of Public Notice, dates and location of postings of posting; III.G—Copy of amended Reasonable Accommodation policy, dates of training to staff on Reasonable Accommodation Policy; Within 270 days from date of execution: III.E—Copy of four-factor LEP analysis, and Language Assistance Plan if applicable. III-H—Copy of revised Needs Assessment and Transition Plan. Copy of sample contracts to sub-recipients requiring that programs or activities conducted with CDBG funds will be provided in accessible locations. V. EFFECTIVE DATE AND DURATION A. The effective date of this Agreement is the date of the last signature on the signature page. This Agreement shall remain in effect for a three year period following the date of execution, unless the Department determines that the Agreement must be revised or extended based on its review of the Recipient's performance under the Agreement and notifies the Recipient to this effect prior to the expiration date. B. Failure to carry out the terms of this Agreement may result in suspension or termination of or refusal to grant or to continue Federal financial assistance, or other actions authorized by law.
