<br />Wellness Initiative <br />The company is committed to improving the health of our employees. That’s why we decided to use the <br />recent Medical Loss Ratio rebate to launch a new wellness initiative focusing on a significant health <br />problem for our employees including Zika Virus Infection and Prevention of Breast Cancer and Diabetes <br /> Insurance- Basic Life and Accident <br />The Basic Term Life Insurance and Personal Accident Insurance (PAI; also known as AD&D) benefits <br />insurer is Life Insurance Company of North America (Cigna). This coverage provides group personal <br />accidental insurance (PAI) to those who are eligible. Basic PAI coverage provides benefits for total <br />disability or death occurring within 365 days of a covered accident, or for the accidental loss of a limb or <br />eyesight. (PAI coverage may also be known as Accidental Death and Dismemberment or "AD&D" <br />coverage.) With basic PAI coverage, employees are covered on the job or off the job, at home or away <br />from home. <br /> <br />Voluntary Benefits <br />For additional peace of mind and to provide an added level of protection for employees and their family <br />when life’s unexpected crises occur, the company offers the following voluntary (employee-paid) <br />benefits <br /> Voluntary Group Universal Life Insurance, Voluntary Personal Accident Insurance, Voluntary Long- <br />Term Disability Insurance, Voluntary Short-Term Disability Insurance <br /> <br />Investment- 401(k) Retirement Savings <br />A 401(k) plan is the tax-qualified, defined-contribution pension account defined in subsection 401(k) of <br />the Internal Revenue Code. [1] Under the plan, retirement savings contributions are provided (and <br />sometimes proportionately matched) by an employer, deducted from the employee's paycheck before <br />taxation (therefore tax-deferred until withdrawn after retirement or as otherwise permitted by applicable <br />law), and limited to a maximum pre-tax annual contribution of $18,000 (as of 2015). The company will <br />offer this benefit and a matching benefit to help employees prepare for retirement. <br /> <br />529 College Savings Plan Information <br />A 529 plan is a tax-advantaged investment vehicle in the United States designed to encourage saving <br />for the future higher education expenses of a designated beneficiary. 529 plans are named after section <br />529 of the Internal Revenue Code 26 U.S.C. § 529. While most plans allow investors from out of state, <br />there can be significant state tax advantages and other benefits, such as matching grant and <br />scholarship opportunities, protection from creditors and exemption from state financial aid <br />calculations for investors who invest in 529 plans in their state of residence. <br /> <br />Additional Benefits: <br />Adoption Assistance Program <br />This program reimburses an eligible employee up to $1,500 for qualified adoption expenses associated <br />with the adoption of an eligible child, whether through private adoption or an adoption through a licensed <br />adoption agency. The company intends that its Adoption Assistance Program qualify as a plan <br />providing adoption assistance under Internal Revenue Code (IRC) Section 137 and that <br />reimbursements made under the Adoption Assistance Program be eligible for exclusion from <br />participating employees' income under IRC Section 137(a). <br /> <br />Commuter Benefits <br />The Commuter Benefits Program allows participants to save up to 40% on transportation costs by <br />paying for mass transit and/or parking expenses with pretax dollars. <br />422
