<br />Consulting Services Agreement between City of San Leandro and 09/08/2016 <br />Safe Moves for Traffic Safety Education Programs for Youth and Senior Adult <br /> Exhibit A – Page 10 of 15 <br />expectations, identification of stakeholders and priorities, and administrative issues, such as invoicing, <br />reporting and other relevant project needs. Safe Moves will prepare materials including agenda for the <br />meeting as well as meeting minutes with action items outlined. <br /> <br />Database <br />Safe Moves will create a database comprising of community centers, senior centers, health care facilities, <br />libraries and religious-base institutions. This listing will include a profile featuring the following information: <br /> <br /> Name of program coordinator/contact <br /> Address, telephone and fax number <br /> Description of services provided <br /> Language needs <br /> Bicycle and pedestrian collision data for the immediate area <br /> <br />Program Introduction Letter and Support Material <br />Safe Moves will prepare a Program Introduction Letter and Support Material for City Staff to review. The <br />letter and material will include all required language and logos of the City of San Leandro and California <br />Office of Traffic Safety. Safe Moves will send the letter and support material to each principal with follow up <br />calls made to answer questions and schedule programs dates. If necessary and/or requested, Safe Moves <br />will schedule meetings or presentations. <br /> <br />In addition, Safe Moves will prepare the following materials in English, Spanish and Chinese to support the <br />Program: <br /> <br />Advertising/Promotion Printed Material <br />Safe Moves will produce the following material to advertise and promote the Program: <br /> <br />Bicycle and Pedestrian Safety Handouts <br />Safe Moves will prepare age-appropriate materials for participants. <br /> <br />Transportation Surveys <br />Surveys will be conducted schools and sites serving older adults to determine the site transportation <br />choices. Survey results will include in questionnaire with in the Month End Reports. <br /> <br />Website Content <br />Safe Moves will provide City Staff with program information and materials to be posted on the City and <br />service providers websites. This material will include reference materials, older adult resource material and <br />on-going awareness of program elements. <br /> <br />All materials will be produce to accommodate the language needs of the recipients. <br /> <br />Master Calendar <br />After contacting identified sources, Safe Moves prepare a master calendar. The master calendar will <br />effectively distribute programs uniformly to reach participants of different ages, geography, ethnicity and <br />socioeconomic backgrounds with an emphasis on high collision areas.
