EXHIBIT A <br />CITY OF SAN LEANDRO <br />WATER POLLUTION CONTROL PLANT REHABILITATION PROJECT <br />(PROJECT NO. 08-593-52-239) <br />PROJECT CLOSEOUT SUPPORT SERVICES <br />SCOPE OF SERVICES <br />INTRODUCTION <br />The City of San Leandro (City) owns and operates a 7.6-mgd rated average dry weather flow (ADWF) <br />Water Pollution Control Plant (WPCP) located in San Leandro, California. Carollo Engineers (Consultant) <br />performed design services for the WPCP Rehabilitation Project, to restore the capacity and function of the <br />WPCP with improvements to various existing facilities and by adding new facilities. Construction of the <br />project was recently completed by S.J. Amoroso Construction Co., Inc. Construction Management of the <br />project was provided by the City staff with assistance from Carollo Engineers. <br />SCOPE OF SERVICES <br />Consultant's Scope of Services for this CSA, herein, incorporates incomplete tasks carried over from the <br />expired CSA dated August 2, 2011. <br />In addition, Consultant's Scope of Services includes providing engineering services during project closeout <br />phase to the City's staff. <br />Task 1— As -needed Assistance with Project Closeout (New). <br />Consultant shall provide as -needed assistance to the City to close-out the project. Consultant shall <br />primarily assist City with closing out yet to be resolved Potential Contract Change Orders(CCOs) with the <br />Contractor. Consultant shall prepare for and attend at least one project management (PM) level CCO <br />meeting every week. Consultant shall also assist the City to prepare for and attend one Executive level <br />CCO meeting every month. Additionally, Consultant shall assist the City with interpreting provisions of the <br />construction documents for clarity when requested, etc. <br />Assumptions: <br />City will lead all project closeout tasks. The consultant shall provide support when requested by the City. <br />Task 2 - Record Drawings (Carry -Over Task from Expired CSA) <br />Consultant shall prepare record drawings of the project to incorporate changes made during the <br />construction period. Consultant and Consultant's project architect shall prepare record drawings using <br />Consultant's standard format to include changes made during the construction period. <br />Information Provided by City: <br />Consulting Services Agreement between Project No. 08-593-52-239 <br />City of San Leandro and Carollo Engineers, Inc.
