In addition to the pay established in Section 1, the City shall pay medical and dental plan costs for salaried officers <br />and full-time probationary and permanent status employees as follows: <br />EVET3 :. .T <br />Effective March 1, 1996, the City will establish an IRS 125 Plan (cafeteria benefit plan) which will <br />provide the following elements: <br />1. Pretax payments of employee contribution toward premium; <br />2. Medical Care Flexible Spending Account up to a $1,000 maximum per benefit plan year; <br />3. Dependent Care Flexible Spending Account up to a $5,000 maximum per benefit plan year; <br />4. A "Core Flex" medical plan; <br />5. A payment of One Hundred Fifty and no/100 Dollars ($150.00) per month for employees <br />electing not to enroll in the medical and dental plans OR a payment of One Hundred and <br />no/100 Dollars ($100.00) per month for electing not to enroll in the medical plan only. <br />Effective January 1, 1998, the City will contribute the following monthly amounts <br />toward the election of benefits in the plan medical benefits: <br />Employee <br />Employee Plus One <br />Employee Plus Family <br />$138.13 per month <br />276.26 per month <br />367.43 per month <br />Effective January 1, 1999, the City will contribute the following monthly amount toward the election <br />of medical benefits: <br />Employee <br />Employee Plus One <br />Employee Plus Family <br />$148.13 per month <br />286.26 per month <br />377.43 per month <br />Employees electing to enroll in the 14eaM Ne CaliforniaCare HMO or Blue Cross Prudent Buyer <br />Plan will be required to contribute the difference between cost of the premium for the Kaiser Plan and <br />the actual cost of the medical plan premium. Such employee contribution will be on a pre-tax basis as <br />specified above. <br />19 <br />
