CONDITIONS OF APPROVAL <br /> PLN17-0041 —2192 East 14' Street <br /> MITC East Bay (Applicant) <br /> YN Lian Tao & Nancy Tao (Property Owners) <br /> L COMPLIANCE WITH APPROVED PLANS <br /> 1. The project shall comply with Exhibits A through F, dated August 17, 2017, except as <br /> hereinafter modified. Exhibits are on file at the City of San Leandro, Community Development <br /> Department, 835 East 14th Street, San Leandro, California 94577. <br /> Exhibit A—Site&Landscaping Plan(Sheet Al) <br /> Exhibit B—Reflected Ceiling Plan(Sheet A-IA) <br /> Exhibit C—Existing Floor Plan Enlarged Toilet Plans,Plumbing Fixture Calcs&Bldg. Elevations <br /> (Sheet A2) <br /> Exhibit D—Trash Enclosure Details(Sheet A3) <br /> Exhibit E—ADA Parking Details&Door Schedule(Sheet A4) <br /> Exhibit F—Administrative Review Letter <br /> 2. Any change to the project design, including materials or colors, or any change to the <br /> Administrative Review Permitted Use Conditions, including, but not limited to increasing the <br /> member of participants or increasing the hours of operation shall be submitted in writing and <br /> shall be subject to the review and approval of the Zoning Enforcement Official who may <br /> administratively approve the changes. <br /> 3. The Applicant and Property Owner shall be responsible for assuring that any successor in <br /> interest who assumes responsibility for this zoning approval is informed of its terms and <br /> conditions. <br /> 11. PERMITTED USE <br /> 1. This Administrative Review is an approval to permit the operation of an adult day care facility, <br /> for a maximum of 70 participants, at 2192 East 14th Street, Assessor's Parcel Number 077- <br /> 0571-038-00. <br /> 2. The adult day care facility's operational hours shall be between the hours of 7:00 a.m. and 4:00 <br /> p.m. Monday through Friday, with programming hours between 8:30 a.m. and 2:00 p.m. <br /> 3. These conditions for approval shall be prominently posted in the day care facility and shall be <br /> distributed to all staff and participants/caregivers or guardians or participants at the facility. <br /> III. ON-SITE IMPROVEMENTS <br /> 1. Applicant shall maintain access and internal circulation from the East 14th Street entrance to the <br /> property. <br /> 2. Applicant shall provide adequate landscaping on the East 14th Street frontage. Prior to issuance <br /> of building permits,the Applicant shall submit final landscape and irrigation plans for the review <br /> and approval of the Zoning Enforcement Official. Said plans shall include all setbacks and <br /> planter adjacent to the sidewalk and driveway. At a minimum, the planting plan shall include <br /> shrubs with a minimum size of five gallons and the ground cover shall be one gallon or from <br /> Agreement to Conditions September 11,2017 <br /> PLN 17-0041 Page 3 of 6 <br />
