<br /> <br /> <br />Young Consulting Response: <br />San Leandro’s Information Technology RFQ for Accela Development Services <br />3-15 <br />Grading Permit <br />Several Items need to be changed in the Grading Permit report: <br />1. Remove Danny Gutierrez’s name and contact information from the <br />Grading Permit report. In its place, add “PLEASE CALL (510) 577-3308 TO <br />SCHEDULE A PRE-CONSTRUCTION MEETING AND INSPECTIONS, AT LEAST 24 HOURS <br />PRIOR TO WORK”. <br />2. The fee field in the Grading Permit report should include the permit fee, <br />review & inspection deposit, performance bond, erosion control deposit, <br />and a sum total. <br />3. Add an "Issued By" field to the Grading Permit report that has the user <br />name of the person who issued the permit. 4. Include the Customer <br />Numbers for each of the different deposits (review and inspection, <br />performance, erosion control) in the list of payments. <br />E/T Priority #4. The Banner Permit fee for an Encroachment Permit should not <br />automatically add the fee to the Encroachment Permit report. Sometimes, <br />the Banner Permit fee is waived. The fee should only be added when a <br />“Banner Encroachment Permit” fee is added to the record. Need to also <br />revisit how Blanket CN's are fees. <br /> <br />2. Expand the text box for the "Description of Work" on the Encroachment <br />Permit report by narrowing the "Customer #" box. When the descriptions <br />get too long, the signature lines get pushed onto a second page, pushing the <br />"General Provisions" onto a third page. <br /> <br />3. Remove shading from the Encroachment Permit hotline box and from the <br />"Traffic Control Required" box. <br />4. Add the permit number, in large font, to the upper right corner of the <br />report. <br />E/T Priority #5.
