Please inquire <br />in advance of completing work. <br />b.Reimbursement claim forms: <br />Forms are available on DBW’s website under the Grants & Loans tab under the <br />appropriate link for SAVE, AWAF and/or VTIP (until further notice, use the SAVE, <br />AWAF or VTIP reimbursement forms as needed for each specific type of removal): <br />Grantee must sign and date each reimbursement claim form in blue ink and submit <br />with the following documents to DBW: <br />c.Invoices from service providers, contractors and/or subcontractors to Grantee: <br />Invoices must contain the following: <br />1. Name and address of Grantee <br />2. Contract or invoice number <br />3. Description of service performed <br />4. Date the service was performed <br />5. Location of each service <br />6. Vessel name, CF# or HIN# if available; otherwise, description of vessel <br />d.Proof of payment for all invoices. <br />The following acceptable forms of proof are: <br />1. Cancelled check (with bank’s cancelled stamp on back of check copy) <br />2.Credit card statement with charge and payment posted, along with copy of <br />charge slip <br />3. Receipt from service provider showing payment received <br />4.Receipt from service provider posted as “Paid” with authorized signature or <br />indicating a zero balance. <br />e.10% in-kind match contribution statement: <br />1.If Grantee is matching the 10% requirement with in-kind services, complete <br />the following form located on DBW’s website and submit with each claim: <br />http://www.dbw.ca.gov/PDF/Funding/DPR265.pdf <br />2.Net, hourly rates only will be accepted.
