Procurement Contract No. 16383 <br />EXHIBIT A <br />SCOPE OF SERVICES <br />1. CONTRACTOR shall provide outreach to unsheltered individuals as more fully <br />described below. <br />a. This Exhibit has been drafted to include the requirements contained in the <br />Unsheltered Homeless Immediate Impact Grant Request for Proposals, including <br />any addenda, specifically including the "Program" section of the RFP, the <br />proposal response of CONTRACTOR (Response), and additional services that <br />the COUNTY obtained through negotiations, if any. In the event of any conflict <br />(direct or indirect) among any of the exhibits, the RFP and the Response, the <br />more stringent requirements providing the COUNTY with the broader scope of <br />services shall have precedence, such that this Exhibit A including all <br />attachments, the scope of work described in the RFP and the scope of work <br />described in CONTRACTOR's proposal shall be performed to the greatest extent <br />feasible. <br />b. The RFP and Response may be relied upon to interpret this Contract and shall be <br />applied in such a manner so that the obligations of the CONTRACTOR are to <br />provide the COUNTY with the broadest scope of services for the best value. <br />C. CONTRACTOR will make best efforts to collect and input data included in <br />Reporting Requirements Section 3.d.i below at 100% data quality. <br />d. "Homeless" as defined by HUD, means an individual or family with a primary <br />nighttime residence that is a public or private place not designed for or ordinarily <br />used as a regular sleeping accommodation for human beings, including a car, <br />park, abandoned building, bus or train. <br />2. Specific Requirements: <br />a. CONTRACTOR'S team will consist of the following Key Personnel, as applicable <br />during the contract term: <br />Jeanette Dong <br />I City <br />of San Leandro Recreation and Human Services Director <br />Officer Joseph Camarillo <br />I City <br />of San Leandro Police Department <br />The COUNTY approves of CONTRACTOR'S use of the following subcontractors: <br />Liz Varela Executive Director, Building Futures for Women and Children <br />Sabrina Thomas Program Director, Building Futures for Women and Children <br />Exhibit A <br />Page 1 of 3 <br />
