Procurement Contract No. 16383 <br />Jose Garcia I Street Outreach Manager, Building Futures for Women and Children <br />CONTRACTOR agrees that it shall not transfer or reassign the individuals identified <br />above as Key Personnel or substitute subcontractors without the express written <br />agreement of COUNTY, which agreement shall not be unreasonably withheld. Should <br />such individual or individuals in the employ of CONTRACTOR no longer be employed <br />by CONTRACTOR during the term of this Agreement, CONTRACTOR shall make a <br />good faith effort to present to COUNTY an individual with greater or equal <br />qualifications as a replacement subject to COUNTY's approval, which approval shall <br />not be unreasonably withheld. <br />b. CONTRACTOR shall provide street outreach services to unsheltered individuals <br />within the City of San Leandro, that includes: <br />i. Outreach to at least 5 total unduplicated unsheltered individuals every day, <br />providing information about housing and shelter options and supportive <br />services and gathering information about the individuals to assist in <br />connecting the individuals with resources. The 5 individuals can include both <br />people who were contacted on previous days as well as completely new <br />contacts. <br />ii. Ensuring that unsheltered individuals complete intakes that are entered into <br />the countywide Homeless Management Information System and that they are <br />assessed for Coordinated Entry. <br />C. The COUNTY is currently working to establish a homeless Coordinated Entry <br />(assessment and referral) System (CES), which includes the development of <br />Housing Resource Centers. As these systems are put into place, CONTRACTOR <br />is expected to participate in and coordinate with the system(s). <br />d. Minor modifications may be made to the numbers to be served and the budgeted <br />amounts listed in this section upon written approval of the County Housing and <br />Community Development Director. <br />3. Reporting Requirements: <br />a. CONTRACTOR shall measure impact of the project using the reporting requirements <br />below. <br />b. CONTRACTOR shall submit monthly reports no later than 15 days after the end <br />of the month being reported on during the first year of the contract period. <br />C. CONTRACTOR shall submit a final narrative within thirty (30) days at the end of <br />Exhibit A <br />Page 2 of 3 <br />
