Procurement Contract No. 16383 <br />the contract period, in a format approved by COUNTY containing, at a minimum, <br />the information in Exhibit A. The report shall include a narrative accounting of the <br />progress achieved toward the Scope of Work objectives, and the following <br />unsheltered individual information using aggregate reporting. <br />d. MONTHLY REPORT SUMMARY <br />i. Number of Individuals Served: <br />1. Aggregate Unsheltered Individual Characteristics, which is represented <br />within a pdf-copy of Annual Performance Report (APR), (developed <br />and maintained by the HMIS software vendor) and extracted by the <br />CONTRACTOR. <br />2. Number of new unsheltered individuals entered into HMIS during the <br />reporting period. <br />ii. Quality of Program and Service Provided: <br />1. Change in the number of unsheltered individuals engaged weekly with <br />local service providers provided with housing, shelter, laundry, <br />showers, medical care, and mental health services. <br />iii. Program Impact and Outcomes: <br />1. Number and percentage of unsheltered individuals who have been <br />served by outreach services and subsequently entered shelter. <br />2. Number and percentage of unsheltered individuals who have been <br />served by outreach services and subsequently entered permanent <br />housing. <br />3. Number and percentage of unsheltered individuals connected to or <br />maintaining enrollment in mainstream benefits, by type of benefit. <br />4. Number and percentage of unsheltered individuals who follow up on <br />housing referrals. <br />e. CONTRACTOR shall make good faith efforts to provide other information, as <br />requested by the COUNTY, in a timely manner. <br />4. The approval of COUNTY to a requested change shall not release CONTRACTOR <br />from its obligations under this Agreement. <br />Exhibit A <br />Page 3 of 3 <br />
