EXHIBIT A <br />SCOPE OF SERVICES <br />Contractor shall produce and deliver meals to homebound seniors Monday through Friday and provide <br />weekend and holiday meals as needed. The delivery driver provides a safety check in visit for the senior <br />with every delivery. Drivers observe if a senior is declining or needs urgent assistance. SOS program staff <br />address the need for additional services, obtain emergency services, and alert far away family members if <br />further intervention is needed. The most common issues SOS provides assistance with are transportation <br />and procuring in home support services. <br />Contractor shall provide the following: <br />Deliver 44 - 47 meals per month to San Leandro seniors at a cost of $9.00 per meal. <br />Serve 24 unduplicated San Leandro seniors. <br />Contractor shall meet the following timeline: <br />From July 1 - December 31, 2018 serve a minimum of 278 meals to San Leandro seniors. <br />From January 1 - June 30, 2019 serve a minimum of 278 meals to San Leandro seniors <br />Consulting Services Agreement between City of San Leandro and Last revised July 1, 2018 <br />SOS/Meals on Wheels for Community Assistance Program (CAP) Grant Exhibit A — Page 1 of 1 <br />
