EXHIBIT A <br />SCOPE OF SERVICES <br />Contractor shall provide shelter services for women and children inclusive of parent education groups, <br />intake and assessment, therapy and case management. Contractor shall serve a minimum of 40 persons <br />during the duration of this agreement. Persons served must be connected to San Leandro through <br />residency, work, school, family or childcare. <br />Services shall include a Family Clinician (FC) to address educational and developmental issues by <br />conducting an ASQ (ages and stages questionnaire) with the parent and child. A harm and abuse <br />assessment will be conducted with the parent and child individually. The FC shall provide the following <br />services: <br />- Children's Individual Group Therapy and Educational Play: Weekly therapy and educational <br />groups that focus on healing from trauma and developing good developmental behaviors. <br />- Age and Stages Questionnaire for each child: Standardized assessment for needs and <br />goals, information shall be used to coach parents and set goals for proper development. <br />- Tutoring Program: Tutoring for children twice per week, grades Kindergarten through High <br />School. <br />- Parent and child bonding activities and field trips. <br />- Advocacy for educational and developmental delay services. <br />Contractor shall meet the following timelines: <br />July — September 2018: <br />Number of Women and Children served within shelter: 62 <br />Number of Successful Parenting Education Groups: 13 <br />Number of Ages & Stages Questionnaires conducted: 20 <br />Number of Children attending the Children's Program: 40 <br />Number of therapy sessions: 150 <br />Number of participants served that have a direct connection with San Leandro through residency, work, <br />school, family or child care: 10 <br />October — December 2018: <br />Number of Women and Children served within shelter: 62 <br />Number of Successful Parenting Education Groups: 13 <br />Number of Ages & Stages Questionnaires conducted: 20 <br />Number of Children attending the Children's Program: 40 <br />Number of therapy sessions: 150 <br />Number of participants served that have a direct connection with San Leandro through residency, work, <br />school, family or child care: 10 <br />Consulting Services Agreement between City of San Leandro and Last revised July 1, 2018 <br />Ruby's Place for Community Assistance Program (CAP) Grant Exhibit A <br />Page 1 of 2 <br />
