CONDITIONS OF APPROVAL <br />PLN18-0080 — 2125 Williams St. <br />Hanh Giang, Cozy Nest ADHC, LLC. (Applicant) <br />and <br />Randy Jones, 2101 Williams Associates, LLC. (Property Owner) <br />1. The project shall comply with Exhibits A000 through A800, attached to the approval letter dated <br />May 1, 2019, except as hereinafter modified. Exhibits are on file at the City of San Leandro, <br />Community Development Department, 835 East 14th Street, San Leandro, California 94577. The <br />approved exhibits include the following: <br />• Exhibit A095 Site Plan (Proposed Upgrades) — dated March 1, 2019 <br />• Exhibit A300 Proposed Plan — dated March 1, 2019 <br />• Exhibit A311 Exterior Elevations — dated March 1, 2019 <br />• Operational Plan — dated November 26, 2018 <br />• Cozy Nest Portfolio — Undated <br />• Parking Analysis — dated October 12, 2018 <br />2. Any change to the site plans or building shall be subject to the review and approval of the Zoning <br />Enforcement Official, who may administratively approve the changes or refer them to the <br />Planning Commission and Board of Zoning Adjustments in accord with San Leandro Zoning <br />Code Section 5-2104. <br />3. The applicant and property owner shall be responsible for assuring that any successor in interest <br />who assumes responsibility for this zoning approval is informed of its terms and conditions. <br />II. PERMITTED USE <br />4. This Administrative Review is an approval to permit the operation of an adult day care with <br />ancillary support uses, for a maximum of 300 participants and averaging 200 participants in any <br />shift, at 2125 Williams Street, Assessor's Parcel Number 077A 0645 005 01. Any changes in <br />licensure, ownership or expansion of use shall require a new Administrative Review application. <br />5. The adult day care facility's operational hours shall be between the hours of 6:00 am and 8:00 <br />pm. These hours may be adjusted with prior approval from the Zoning Enforcement Official. <br />6. These conditions for approval shall be prominently posted in the facility and shall be distributed <br />to all staff and clients/participants or their guardians upon request. <br />Agreement to Conditions May 1, 2019 <br />PLN 18-0080 Page 3 of 7 <br />
