- 14 - <br />CDBG Agreement with Davis Street Family Resource Center (Basic Needs Services) - FY2018-2019 <br />EXHIBIT A <br /> <br />SCOPE OF WORK <br /> <br />Term of Contract: July 1, 2019 – June 30, 2020 <br /> <br />Agency: Davis Street Family Resource Center <br /> <br />Address: 3081 Teagarden Street <br /> San Leandro, CA 94577 <br /> <br />Contact Person: Rose Padilla Johnson, Executive Director <br /> <br />Phone: 510 347-4620 <br /> <br />Project Name: Family Support Services – Basic Needs Services <br /> <br /> <br />PROJECT GOALS AND OBJECTIVES <br /> <br />The Family Support Services Program will provide “Basic Needs” services to San Leandro community members. <br />This includes emergency food and clothing and other services. The provision of emergency services to low- <br />income people in San Leandro enables these individuals and families to move out of poverty and into self - <br />sufficiency. <br /> <br />The DSFRC Community Counseling Program is an outpatient mental health clinic providing the full spectrum of <br />mental health services across the lifespan. This program specializes in providing culturally relevant intervention <br />(individual, couples, family, and group modalities) and prevention services to enhance the overall qualit y of life <br />for clients. Crisis, short-term, and long-term therapeutic services are provided. Our therapeutic services are <br />designed to work collaboratively with our clients to identify barriers and strategies to enhance their capacity to <br />live full lives. <br /> <br />MAJOR ACTIVITIES <br /> <br />Families will receive five days’ worth of groceries up to twice a month (boxes of groceries include: canned goods, <br />produce, bread, rice and pasta, dairy, and protein items), and emergency clothing and household items. Families <br />also have access to other services provided within DSFRC (housing, childcare, counseling, and medical/dental <br />services, etc.), and they are also provided with linkages to other services among DSFRC local, county, and state <br />resources and partnerships. <br /> <br />DSFRC Community Counseling Program will provide culturally relevant crisis intervention and short-term <br />counseling to individuals, families, and couples across the lifespan. All clients will be provided access to all <br />services within the family resource center-basic needs, medical, and child care services. <br /> <br /> <br /> <br />109
