Please describe the jar test methodology and any other methodology that <br />may be recommended, such as real-world testing or modeling. <br /> <br />2. Complete price list of all polymers that may be recommended. <br /> <br />Because the actual polymer to be used will be determined based on the testing <br />methodology, please provide a price list for all potential polymers. The prices should <br />include all shipping and handling charges and be based on a per -pound basis. The prices <br />should be valid for the specified period for all polymers. <br /> <br />Prices may be quoted as one price for the contract period or may be quoted on a per- <br />year basis. <br /> <br />3. Detail lead-time necessary for ordering additional polymer <br /> <br />Describe how long it will take to ship the polymer to the Water Pollution Control Plant, <br />including locations of manufacturing and distribution centers. <br /> <br />4. References <br /> <br />Please provide contact information for three references for three current municipal <br />customers and three private-sector customers. <br /> <br /> <br /> <br />Non-Collusion Affidavit <br /> <br />The Consultant declares, by signing and submitting a proposal, that the proposal is not made in <br />the interest of, or on behalf of, any undisclosed person, partnership, company, association, <br />organization, or corporation; that the proposal is genuine and not collusive or s ham; that the <br />Consultant has not directly or indirectly induced or solicited any other Consultant to put in a false <br />or sham proposal, and has not directly or indirectly colluded, conspired, connived, or agreed with <br />any Consultant or anyone else to put in a sham proposal, or that anyone shall refrain from <br />proposing; that the Consultant has not in any manner, directly or indirectly, sought by agreement, <br />communication, or conference with anyone to fix the proposal price of the Consultant or any <br />other Consultant, or to fix any overhead, profit, or cost element of the proposal price, or of that <br />of any other Consultant, or to secure any advantage against the public body awarding the <br />contract of anyone interested in the proposed contract; that all statements contained in the <br />proposal are true; and, further, that the Consultant has not, directly or indirectly, submitted his <br />or her proposal price or any breakdown thereof, or the contents thereof, or divulged information <br />or data relative thereto, or paid, and will not pay, any fee to any corporation, partnership, <br />126
