<br />EXHIBIT A <br /> <br />SCOPE OF WORK <br /> <br />Term of Contract: July 1, 2020 – June 30, 2021 <br /> <br />Agency: Spectrum Community Services <br /> <br />Address: 2621 Barrington Court <br /> Hayward, CA 94545 <br /> <br />Contact Person: Lara Calvert, Executive Director <br /> <br />Phone: 510 881-0300 <br /> <br />Project Name: San Leandro Senior Nutrition Meals Program <br /> <br />PROJECT GOALS AND OBJECTIVES <br /> <br />Spectrum Community Services will serve 210 low-income San Leandro seniors—3,675 hot, <br />nutritious meals per year at San Leandro Senior Center and Fargo Senior Apartments. <br /> <br />MAJOR ACTIVITIES <br /> <br />The Senior Nutrition Meals Program will serve nutritious lunches five (5) days a week, up to 248 <br />days per year, at the San Leandro Senior Community Center. The meals are carefully planned <br />and prepared to be delicious, low salt, and nutritionally balanced to supply 1/3rd of the U.S. <br />Recommended Daily Allowance for older adults. <br />Due to COVID-19 pandemic and the Shelter-in-Place, we are starting off the year providing these <br />meals as “pick up & go.” As the rules change and seniors are able to congregate and dine <br />together, we will continue to adjust our delivery method to best meet the health and safety needs of <br />San Leandro seniors. <br /> <br />A Spectrum Site Coordinator will work with Senior Nutrition Program staff, San Leandro Senior <br />Center staff, and volunteers to serve the meals. The Senior Nutrition Program staff or volunteers <br />welcome each participant for lunch, get them signed-in, register new participants, and receive <br />voluntary donations. No senior is ever turned away for inability to pay the suggested donation. To <br />protect participants from any embarrassment of not being able to donate, all donations are <br />anonymous, not tracked by person. Spectrum maintains records of donation amounts received at <br />the lunch site, not by participants. <br /> <br />As part of Spectrum’s Senior Nutrition Program, staff make quarterly presentations on healthy <br />living and nutrition and take questions from program participants.
