<br />Performance will be measured by the number of meals served and the number of San Leandro <br />seniors who participate in the program, with data available on a monthly, quarterly and annual <br />basis. During each year of the two-year grant, Spectrum will serve at least 3,675 nutritious meals to <br />at least 210 San Leandro seniors. <br /> <br />Success for this program is defined not only by meeting or exceeding our goals and objectives, but <br />by the quality of the services provided. Evaluation of Spectrum's meal service is on an on-going <br />activity. Daily review of operations and meals served ensure that management is able to identify <br />and correct any potential production or delivery problems before they become significant. <br /> <br />Meals will be carefully planned and prepared to be delicious, low salt, and nutritionally balanced to <br />supply 1/3rd of the U.S. Recommended Daily Allowance for older adults. Every menu is planned by <br />Spectrum’s Registered Dietitian which is in turn reviewed and approved by the Alameda County’s <br />Dietician. Spectrum’s Senior Meals Program is monitored by the Alameda County Area Agency on <br />Aging (AAA), through program, fiscal and nutrition monitoring providing important feedback to <br />project management as staff continues to strive for excellence. Spectrum consistently earns high <br />marks for maintaining high standards in food nutrition, kitchen practices and program operations. <br />The AAA reports are available for review upon request. <br /> <br />Spectrum will track how the COVID-19 pandemic has impacted the Senior Nutrition Meals Program <br />and will report on this at the end of the fiscal year. <br /> <br />TIMELINES <br /> <br />By September 30, 2020, Spectrum Community Services will have accomplished: <br /> <br /> Provide meals to 52 unduplicated individuals at the San Leandro Senior Center (919 hot <br />meals). <br /> <br />By December 31, 2020, Spectrum Community Services will have accomplished: <br /> <br /> Provide meals to 52 unduplicated individuals at the San Leandro Senior Center (918 hot <br />meals). <br /> <br />By March 31, 2021, Spectrum Community Services will have accomplished: <br /> <br /> Provide meals to 53 unduplicated individuals at the San Leandro Senior Center (919 hot <br />meals). <br /> <br />By June 30, 2021, Spectrum Community Services will have accomplished: <br /> <br /> Provide meals to 53 unduplicated individuals at the San Leandro Senior Center (919 hot meals). <br /> <br />REPORTS AND MISCELLANEOUS <br /> <br />2. Agency must submit the City-provided Quarterly Reports within 15 days of the end of the <br />each quarter. The Quarterly Reports shall reflect the number of persons served, and shall also <br />include a narrative section. The reports shall be submitted even if there are no specific numbers to <br />report, and the narrative report should describe why no specific numbers are reported. <br /> <br />3. To ensure timely expenditure of HUD funds, Agency must submit monthly reimbursement <br />claims using the City's invoice or comparable form with documentation of claim including copies of <br />296
