DELIVERABLES REQUIRED OF SELECTED CONTRACTOR <br /> <br />The selected Contractor(s) shall enter into a Non-Professional Services Agreement with <br />the City of San Leandro (see Attachment A) and submit the following items within fifteen <br />(15) days of notice of award: <br /> <br />1. City of San Leandro business license; to be maintained throughout length of <br />contract <br /> <br />2. Copy of Certificate(s) of Insurance and endorsements in compliance with the <br />requirements of Section 4. of Attachment A – Non-Professional Services <br />Agreement and naming the City of San Leandro as an additional insured <br /> <br />3. Completed IRS W-9 tax form <br /> <br />4. Self-verification form which shows compliance with the City of San Leandro Living <br />Wage Ordinance <br /> <br />5. Copies of all Safety Data Sheets (SDS) for all products that may be used in the <br />implementation phase of the contract <br /> <br />6. A copy of the Contractor’s written safety program, IIPP, and signed list <br />documenting safety training of all employees prior to the commencement of the <br />contract <br /> <br /> <br />VIII. CONDITIONS <br /> <br />Permits and Codes <br />The selected Contractor shall comply with all laws, codes, rules and regulations of the <br />State, County, and City, applicable to the work to be performed at the City’s location(s) <br />for the duration of the contract. The Contractor, who shall pay all lawful charges, shall <br />obtain all permits lawfully required. <br />City of San Leandro Living Wage Ordinance <br />This contract may be covered by the City of San Leandro Living Wage Ordinance (LWO). <br />Bidder’s attention is directed to the San Leandro Municipal Code, Title 1, Chapter 6, <br />Article 6. Successful Bidder must submit completed self-certification form and comply <br />with the LWO if covered. <br /> <br />Living Wage Guidelines and the Self-Certification Form are provided as Attachments B <br />and C.
