<br />3 of 53 <br /> <br />REQUEST FOR PROPOSALS FOR PEST CONTROL SERVICES <br /> <br /> <br />The City of San Leandro is soliciting qualified proposals for citywide pest control services <br />in accordance with this Request for Proposals (RFP). The services and tasks to be <br />performed include the furnishing of all labor, materials, tools, equipment, supplies, <br />services, tasks and incidental and customary work necessary to completely perform pest <br />control services at various City locations and facilities. Award resulting from this RFP will <br />be a fixed contract with an initial term of January 1, 2021, through June 30, 2022, with <br />four one-year renewal options. <br /> <br />I. SCOPE OF SERVICES <br /> <br />The scope of work of this contract will include all materials, transportation, labor, <br />equipment, any and all services and materials necessary to perform pest control services <br />and provide a pest-free environment in specified offices, buildings and/or grounds. <br /> <br />Only proposals from qualified pesticide applicator (operator) business duly registered and <br />licensed in the State of California to render such services will be considered. <br /> <br />The selected Contractor is to provide for an environment that is free from, but not limited <br />to, the following pests: <br /> <br />1. Ants and cockroaches <br />2. Spiders <br />3. Fleas and other biting insects <br />4. Bed Bugs <br />5. Termites <br />6. Wasps, hornets, and other stinging insects nesting in City facilities and/or parks <br />7. Mice, rats, and other rodents <br /> <br />All buildings and properties listed on the attached Cost Proposal (Attachment D) sheet <br />shall receive monthly services for ants and roaches. In addition, all buildings will receive <br />service for spiders at least one time per year. Specified facilities will receive bi-monthly or <br />quarterly treatment for bed bugs, and rodents. <br /> <br />The City reserves the right to add or remove facilities listed on the Contract Proposal at <br />any time, for any duration, and for any reason, during the agreement term. The Contractor <br />will be required to quote for service for any additional facilities as requested by the City. <br />Furthermore, the Contractor will be required to deduct costs for any facilities where <br />services is not provided as specified in the Cost Proposal. <br /> <br />All Contractor personnel shall wear a uniform with proper identification whenever working <br />in or around any City facility and service vehicles shall be clearly marked. Contractor <br />personnel shall schedule visits with the Building Supervisor prior to providing services <br />and check in with facility staff prior to performing any inspection or service. The contractor <br />must perform a thorough inspection during routine service. <br />33
