Box 942874, MS 74 <br />Sacramento, CA 94274-0001 <br />LOCAL AGENCY: <br />City of San Leandro <br />ADDRESS: <br />835 East 14' Street <br />Effective Date of Contract: December 19, 2015 <br />San Leandro, CA 94577 <br />Effective Date of Amendment 3: Upon Signature by all <br />Phone: <br />(510) 577-3428 <br />Parties <br />Expiration Date of Contract: June 19, 2022 <br />WHEREAS: Service Contract 75LX293 dated December 19, 2015, provided $340,875 to reconstruct the south median island, <br />reconstruct sidewalk on all quadrants, install detectable warning tiles (tactile strips) on all sidewalk approaches, install pedestrian <br />channelization fencing, install R8-8 on both approaches at Washington Avenue and the Union Pacific Railroad Company's tracks, in <br />the City of San Leandro, CPUC No. 001D-16.50, Federal DOT No. 749743X; and <br />WHEREAS: Amendment No. 1, added $343,738, for a total of $684,613, to accommodate a scope modification to add a median <br />island for vehicle channelization and two pedestrian ramps to meet current ADA standards and extended the expiration date for <br />Service Contract 75LX293, from December 19, 2018, until December 19, 2020; <br />WHEREAS: Amendment No. 2 extended the contract an additional eighteen months to complete construction; <br />WHEREAS: the City has requested an additional $150,000 to acquire right-of-way (ROW) for needed pedestrian improvements <br />and to complete design before the start of construction <br />NOW THEREFORE: <br />An additional $150,000 is added to the contract for a total not -to -exceed amount of $834,613.00. <br />It is mutually agreed by the parties hereto, that all terms and conditions of Service Contract 75LX293, as amended, shall remain in <br />full force and effect, and shall be made a part of this Amendment No. 3. <br />IN WITNESS WHEREOF, the parties have duly executed this contract as of the dates shown below with the effective date <br />shown above. <br />CALIFORNIA DEPARTMENT OF TRANSPORTATION <br />Signature <br />i <br />Name W.
