<br />Consulting Services Agreement between May 1, 2021 – June 30, 2024 <br />City of San Leandro and Du-All Safety, LLC Exhibit A – Page 1 of 5 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br /> <br />MEMORANDUM of AGREEMENT <br />for <br />Environmental Health & Safety Maintenance (EH&S) <br />This Agreement is entered into by and between The City of San Leandro with its <br />principal offices at 835 East 14th St., San Leandro (Client) and Du-All Safety, LLC, <br />located at 45950 Hotchkiss St, Fremont, CA (Du-All Safety) for the purpose of providing <br />environmental health and safety consultation. The undersigned party (hereafter <br />referred to as "Client") hereby agrees to the development and implementation of safety <br />programs, employee training and compliance services by Du-All Safety, not to exceed <br />667 hours, for Fiscal years 2020/21 thru 2023/24, as described in the Services Summary <br />below: <br />I. Services Summary: <br />The following service summary is a menu of support services available to be provided, <br />as requested by the client: <br />a) Provide assessments of, development of, or updates to, written safety programs, <br />policies, and procedures.
