❑ Site Plan <br />• Provide a drawing showing where each feature and support amenity listed in the <br />PROJECT SCOPE/Cost Estimate Form will be located. <br />• Include the function and approximate square footage of each room within <br />buildings that are part of the PROJECT SCOPE, and the approximate total square <br />footage of the buildings. <br />❑ Land Tenure and Site Control <br />If the property is owned in fee simple by the APPLICANT, provide a copy of the <br />• Deed or deed recordation number <br />• Title report <br />• Tract map (if owner's name provided) <br />If the property is not owned in fee simple, provide the lease, easement, joint powers <br />agreement, etc. <br />❑ Sub -leases or Agreements <br />• Provide a list of all other leases, agreements, memoranda of understanding, etc., <br />affecting PROJECT property or its operation and maintenance, or <br />• If this requirement is not applicable, state, "This item is not applicable because <br />there are no subleases or other agreements." <br />❑ California Environmental Quality Act (CEQA) <br />The APPLICANT should check with its local city or county planning agency for CEQA <br />compliance information. <br />• If CEQA is complete: provide the CEQA Compliance Certification Form and its <br />required attachment, filed and stamped by the County Recorder. <br />• If CEQA is not complete: provide a timeline for completion and state "completing <br />CEQA is a Project Scope Item." <br />❑ PROJECT Location Map <br />• Provide a map showing highway and street access to the PROJECT site. <br />7 <br />
