The Subrecipient shall be responsible for submission of interim and final narrative <br />and financial reports on the progress and activities of the project, to the <br />California State Library, using the sample report documents provided by the <br />California State Library. <br />2. All the reports must be current, include all required sections and documents, and <br />must be approved by the Grant Monitor before any payment request can be <br />processed. Failure to comply with the specified reporting requirements may be <br />considered a breach of this Agreement and result in the termination of the <br />Agreement or rejection of the payment request and/or forfeiture by the <br />Subrecipient of claims for costs incurred that might otherwise have been eligible <br />for grant funding. Any problems or delays must be reported immediately to the <br />Grant Monitor. The financial reports shall reflect the expenditures made by the <br />Subrecipient under the Agreement, and may be incorporated into the same <br />reporting structure as the narrative reports. <br />3. The reports shall be submitted by the following dates: <br />Reporting Period Report Due Date <br />July - September 1st Quarter Financial Report Due N/A <br />October - December 2nd Quarter Financial Report Due and <br />Mid Project Program Narrative Report <br />Due <br />May 31, 2022 <br />January - March 3rd Quarter Financial Report Due N/A <br />April - Project End Date Final Financial Report, Expenditure <br />Detail Report and Final Program <br />Narrative Report Due <br />Upon completion of the <br />project but no later than <br />September 30, 2022 <br />60 Days from Project <br />End Date (if end date <br />is extended see <br />extension letter for <br />new dates) <br />Liquidation Financial Report Due (Only <br />required if encumbered funds have <br />not been spent by project end date) <br />October 31, 2022 <br />4. Failure to submit timely reports with the appropriate documentation by the due <br />date may result in rejection of the payment request and/or forfeiture by the <br />Subrecipient of claims for costs incurred that might otherwise have been eligible <br />for grant funding. <br />5.
