<br />Amendment No. 1 to <br />Consulting Services Agreement between City of San Leandro and CSG Consultant, Inc. for Washington <br />Avenue Reconstruction Project Page 1 of 3 <br />AMENDMENT NO. 2 TO CONSULTING SERVICES AGREEMENT BETWEEN THE CITY OF SAN LEANDRO AND <br />CSG CONSULTANTS, INC <br />FOR <br />WASHINGTON AVENUE RECONSTRUCTION <br />PROJECT NO. 2018.4020 <br /> <br /> <br />This Amendment No.2 (“Amendment”) is made by and between the City of San Leandro (“City”) <br />and CSG Consultants, Inc. (“Consultant”) (together sometimes referred to as the “Parties”) as of ________, <br />20__, and amends that certain Consulting Services Agreement (“Agreement”) dated June 30, 2020, <br />between the Parties. <br /> <br />WHEREAS, City and Consultant have executed the Agreement, pursuant to which Consultant has <br />provided certain consulting services to City with regard to the Washington Avenue Reconstruction Project, <br />Project No. 2018.4020, and <br /> WHEREAS, the Parties desire to amend the Agreement to add additional scope of work. <br /> <br />NOW THEREFORE, for good and valuable consideration, the sufficiency of which is hereby <br />acknowledged, the Parties hereby agree to amend the Agreement as follows: <br /> <br />1. Section 1.1 of the Agreement entitled “Term of Services” is hereby amended to extend the <br />term from December 31, 2022, to December 31, 2023; and <br /> <br />2. Section 2 of the Agreement entitled “Compensation” is hereby amended to pay Consultant a <br />sum not to exceed TWO HUNDRED NINETY-FIVE THOUSAND, SIX HUNDRED TWENTY-NINE <br />DOLLARS ($295,629); and <br /> <br />3. Exhibit A of the Agreement entitled “Scope of Services” is hereby amended to as follows: <br /> <br />a) Design storm drain facilities at the intersections of Washington and Thornton St., Williams St. <br />and Castro St. complete and suitable for bidding, including pothole investigations required to <br />verify and address utility conflicts that will impact the proposed SD alignments. Implement <br />traffic control, obtain a no-fee City issued permits and perform restoration, all in association <br />with work required by this additional scope of work. <br /> <br />4. Exhibit B of the Agreement entitled “Compensation Schedule & Reimbursable Expenses” is <br />hereby amended to read: <br /> <br />Task 5a: Utility Potholing <br />Amendment No. 2 $21,964 (new) <br />Original Contract $248,847 <br />Amendment No. 1 $24,818 <br />New PO Total $295,629
