EXHIBIT A <br />SCOPE OF WORK <br />Term of Contract: January 5, 2022 – June 30, 2023 <br />Consultant: REBUILDING TOGETHER OAKLAND | EAST BAY <br />Address: 520 3rd Street, Suite 109 <br />Oakland, CA 94607 <br />Contact Person: Lisa Malul, Executive Director <br />Phone: 510-625-0316 ext. 101 <br />Project Name: Lead-based Paint Assessment & Remediation of ERAP Properties <br />PROJECT GOALS AND OBJECTIVES <br />Conduct lead-based paint assessment of properties that received FY 2020-21 Emergency Rental <br />Assistance Program HUD CDBG-CV funds. These assessments are required for approximately 110 <br />addresses in 88 properties and will include communication with property owners and tenants. <br />Assessments will include written and photographic documentation of the results of those assessments, <br />the scope of work and cost of remediation for those properties found to have lead-based paint, and <br />project management of lead-based paint remediation. See Exhibit E for Lead Safe Housing Rule, 24 CFR <br />35, Subpart K (for CDBG-Funded Households) compliance steps provided by Dale Hagen of Alameda <br />County Lead Hazard Control Division. <br />DESCRIPTION OF SERVICES <br />A. Consultant shall conduct lead-based paint assessments and reports of both the interior and <br />exterior of 110 addresses in 88 properties. The assessments and reports will require the following <br />documentation: <br />•Confirmed Lead-safe Housing Rule Assessment Training of staff conducting this work on <br />behalf of RTO <br />•Written communication with Property Owner/Manager and Tenants to advise of required <br />lead-based paint assessment. <br />•Schedule on-site assessments within the limitations of the current public health protocols <br />of the COVID-19 pandemic. Consultant shall obtain any necessary consent from the <br />Property Owner/Manager and Tenants prior to performing any on-site assessments. <br />•Document on-site assessment both in writing with reference photographs. <br />•Report of addresses/properties that require lead-based paint testing: <br />o Summary report of test results to City staff, <br />o Letter that reports results of test to Property Owners & Tenants. <br />B. Consultant shall report addresses/properties that are over the de minimis amount of lead-based <br />paint that require lead-based paint remediation: <br />•Summary report of required remediation to City staff that includes: <br />o Property address, <br />o Project Summary/Scope of Work for remediation, <br />o Itemized estimated cost of remediation, <br />o Certification Letter. <br />•Letter that summarizes required remediation to Property Owners & Tenants; <br />C. Consultant shall provide project management services to City for properties that require lead- <br />based paint remediation. Consultant shall obtain any necessary consent from the Property <br />Owner/Manager and Tenants prior to performing any remediation. <br />358
