toward medical premiums. <br />b. Medical Flexible Spending Account with a maximum <br />employee pretax contribution of $1,000 per benefit <br />plan year. <br />C. Dependent Care Flexible Spending Account with a <br />maximum employee pretax contribution of $5,000 per <br />benefit plan year. <br />The City will maintain in effect a "core flex" medical <br />benefit plan. The core shall consist of the Kaiser Medical <br />Plan with a five dollar co -payment and the existing MetLife <br />Dental Plan. <br />The City will contribute the following monthly amount toward <br />the election of benefits in the plan: <br />Employee $134.80 <br />Employee +1 $269.59 <br />Family $358.56 <br />During the term of this agreement, these amounts will be <br />adjusted by an amount equal to the change (increase or <br />decrease) in the Kaiser 5 + 5 premium. <br />Employees electing to enroll in the CaliforniaCare HMO Plan <br />C2 or Blue Cross Prudent Buyer Plan will be required to <br />contribute the difference between the above amounts and the <br />actual cost of the medical plan premium. Such employee <br />contribution will be on a pre-tax basis as specified above. <br />The City and Association will continue to meet and confer <br />regarding the implementation of additional voluntary <br />benefits into the benefit plan. <br />Employees electing not to enroll in the core medical plan <br />(i.e. non -enrollment in the medical and dental plan) and <br />demonstrate to the satisfaction of the City of enrollment in <br />another medical plan, shall receive $150 per month for their <br />non -election of medical benefits. In the event both spouses <br />are employed by the City and eligible to enroll in the <br />City's medical benefits plan, one employee may elect not to <br />enroll in the medical plan and will receive the $150 per <br />month payment. <br />29.2 The City shall pay the contributions required by health plan <br />two-party rate for retired City employees who were assigned <br />to classifications represented by the San Leandro City <br />Employees Association and who are currently members of one <br />of the City's three health plans. The City shall contribute <br />to the health plan two-party rate costs until the maximum <br />amount to be contributed the City may not exceed Three <br />Hundred Forty-one Dollars and four cents ($341.04) to the <br />27 <br />
