Kaiser Health Plan, Three Hundred Thirty-two Dollars and <br />seventeen cents ($332.17) to the Blue Cross Health Plan, and <br />Three Hundred Sixty Dollars and ten cents ($310.00) per <br />month to the CaliforniaCare HMO Plan. In the event the <br />amounts required by the health plans exceed the maximum City <br />contribution, such excess amounts shall be paid by the <br />retiree. <br />29.3 Retired employee dependent eligibility for City health plan <br />contribution is conditional upon the active enrollment of <br />the retired employee. If a retired employee moves outside <br />the service area of their medical plan, the retiree will be <br />allowed an opportunity within thirty (30) days of such move <br />to change medical insurance coverage. If a retired employee <br />remarries, the retiree may add the retiree's spouse to the <br />medical insurance coverage at the City's expense. <br />29.4 The parties mutually agreed to explore the possibility of <br />offering to a retiree 65 years of age or older, an optional <br />premium payment to be used toward an individual medical <br />supplement conversion plan, under which retirees may be <br />offered the option of taking a reduced premium contribution <br />by the City prior to reaching age sixty-five (65). The <br />availability of such coverage, the cost thereof, and other <br />related matters is unknown to the parties at this time. <br />However, the parties agree to meet during the term of this <br />agreement to explore such options and alternatives. In no <br />case, shall the City's obligation to pay premiums exceed the <br />sums and/or costs described in Section 29.2. <br />29.5 The City shall contribute a maximum of Sixty-eight Dollars <br />and fourteen cents ($68.14) per month per active employee <br />for current dental coverage provided by MetLife Insurance <br />Company. Any increases in dental insurance premiums during <br />the term of this Agreement shall be paid by the City. <br />29.6 The City shall contribute a maximum of Sixty-eight Dollars <br />and fourteen cents ($68.14) monthly dental plan costs for <br />each retired City employee who was assigned to a <br />classification represented by the San Leandro City Employees <br />Association, and who is currently a member of the City's <br />dental plan, or who was a member of the City's dental plan <br />and retired on or after January 1, 1980. Any increases in <br />dental plan costs during the term of this Agreement shall be <br />split equally between the City and the retired employee. <br />Section 30. Life Insurance <br />The City shall maintain in effect employer paid Term Life <br />Insurance in the amount of Twenty Thousand ($20,000) Dollars with <br />an AD&D benefit. The City shall make available for employees, the <br />ability to purchase additional life insurance, at no cost to the <br />City, subject to the requirements and rules of the insurance <br />carrier.
