EXHIBIT A <br />SCOPE OF SERVICES <br />1. AGREEMENT TERM. The term of this agreement shall be for the period of <br />July 1, 1996 through June 30, 1997 or until all CDBG/Community Assistance funds <br />authorized by this agreement are expended. <br />2. USE OF FUNDS. The CDBG/Community Assistance funds approved under this <br />Agreement shall only be used in paying service related fees. <br />3. DESCRIPTION OF SERVICES. The services to be provided are the following: <br />Provide services which include gang prevention/intervention counseling. <br />4. PROGRAM GOALS/COMPLETION SCHEDULE. Subgrantee shall counsel thirty (30) <br />children ages 9 - 15 on gang prevention during the program year. <br />5. GOAL AND BUDGET AMENDMENTS. Any amendments to the services provided or <br />project budget must be requested in writing and approved in writing by the City <br />Manager. <br />6. REPORTING. Program goals, monitoring conferences, and customer surveys <br />shall be the basis of measuring performance. Subgrantee shall submit quarterly <br />activity reports by the last day of the month following the end of each calendar <br />quarter. Such reports shall be in the format provided by the City and shall <br />include for each activity: <br />total number of assisted households; <br />description of services provided; <br />total units of service completed; and <br />client data for each head of household: ethnicity, gender, income <br />level, age, and disability. <br />7. INCOME ELIGIBILITY. Assistance shall be provided to eligible households <br />according to the HUD guidelines to be provided by the City.
