6. REPORTING. Program goals, monitoring conferences, and customer surveys shall be the basis of <br />measuring performance. Subgrantee shall submit quarterly activity reports by the last day of the month <br />following the end of each calendar quarter. Such reports shall be in the format provided by the City and <br />shall include for each activity: <br />total number of assisted households; <br />description of services provided; <br />total units of service completed; and <br />client data for each head of household: ethnicity, gender, income level, age, and disability. <br />7. INCOME ELIGIBILITY. Assistance shall be provided to eligible households according to the <br />HUD guidelines to be provided by the City. Subgrantee shall verify eligibility for all program participants. <br />8. DOCUMENTATION AND RECORD -KEEPING. FAMILY SERVICE OF SAN <br />LEANDRO shall maintain all records required by the federal regulations specified in 24 CFR Part <br />570.506, and that are pertinent to the activities funded under this agreement. Such records shall be <br />preserved for a minimum of three years following termination of all activities funded under this agreement, <br />or after the resolution of all Federal audit findings, whichever occurs later, and shall include but are not <br />limited to: <br />1) documentation of the eligibility of program recipients; <br />2) documentation of compliance with fair housing and equal opportunity policies; <br />3) financial records required by 24 CFR Part 570.502 and OMB Circular A-110; and <br />4) other records to document compliance with Subpart K of 24 CFR 570. <br />Exhibit A; FAMILY SERVICE SAN LEANDRO <br />Page 2 of 3 <br />9/26/88 Rev.; <br />1/10/90 Rev. <br />
